Dear Officer Salahuddin:

As noted in previous correspondence to you and/or our attorney, the continued use of
your sick time allotment will deplete your sick time by the midpoint of your August 21,
2010 shift. Thereafter, you are scheduled to work A-Shift on August 22, 2010. Prior to
coming to work on August 22, 2010, you should contact Lieutenant Williams to schedule
training so that you can qualify with your department firearm, as well as review new
policies and procedures that have been introduced into the department. You must
complete this training prior to your resuming work on your shift. If time is an issue,
please speak with Lieutenant Williams about the earliest possible dates.

Additionally, as requested in the past, you are required to produce a letter from a licensed
physician indicating the diagnosis and prognosis of your illness to Lieutenant Williams,
as is standard procedure. This physician’s letter must include your release to full duty.
