Q: What is your full name, current address, phone number that you wish to be contacted at, date
of birth, social security number, driver's license number, and state of issue?

A: It’s Paul Jones and I live at 666 Devils Way, Provo, UT 84604. I would like to be contacted
on my cell phone. The number is 801-224-6920. I was born on May 15, 1975 and my social is
123-45-6789. Here is my driver’s license for the rest of the info as well as verification of my
address.

Q: Why are you seeking the services of an attorney today? How can we help you?

A: I would like to have the alimony that I pay to my ex-wife reduced or eliminated.

Q: How much is your support payment?

A: It’s $500 per month.

Q: When was this ordered?

A: Our final decree was entered on June 1, 2007.

Q: What is the duration of the spousal maintenance?

A: It’s for 10 years. It will end on June 1, 2017.

Q: What is your ex-wife’s name?

A: Jenny Jones.

Q: Did you have any children with Jenny and who has custody if there are children?

A: No kids.

A: No.

Q: Question: What is your current marital status?

A: I married Sue Spielman on July 1, 2007 and this is my second marriage.

Q: Why are you asking the court to reduce or eliminate your support payment?

A: Sue and I had a baby that was born prematurely on May 1, 2008, and there are many
complications. My insurance does not cover all of his care which is very expensive. His name is
Joseph.

Q: Does your wife work and does she have insurance?

A: No. My wife is not working because she is at the hospital day and night with the baby.

Q: Does she have any kind of income and insurance to help to help cover the additional medical
expenses?

A: She is collecting unemployment benefits. She gets $600 per month from that with no
insurance. But those benefits will run out on September 15th.

Q: How much are the additional medical expenses costing you?

A: It’s about $1,000 per month. Maybe a little more.

Q: What is your current gross income and employer?

A: I have worked at ACME Car Care since 2006. I was working there when I was married to
Jenny. I make $16.50 per hour.

Q: How many hours do you work?

A: 40 hours a week.

Q: Do you or Sue have any other income?

A: No.

Q: How much gross pay were you making when the final decree was entered?

A: I was making $15 per hour.

Q: Was your ex, Jenny, working at the time?

A: No. She was a full-time student. She graduated about 6 months ago. Jenny received a B.S.
degree in Tax Accounting and she has a CPA too.

Q: Do you know if Jenny is employed?

A: Yes. She works at the Thompson Accounting Firm as a junior tax accountant.

Q: Do you know what her current gross earnings are?

A: She makes about $30,000 per year.

Q: Did she re-marry?

A: No.

Q: Does she share expenses with someone?

A: Not that I know of.

Q: Does she support any children from a previous marriage or relationship?

A: No.

Q: Did you bring any documentation or evidence to support your claims?

A: Yes. I brought the medical bills and a letter from the Labor Department.

Q: How did you find out that she holds a B.S. in Tax Accounting and a CPA?

A: There was an announcement in the paper when she joined Thompson. It also stated that she
received her CPA and the type of degree she held.

Q: Do you have that with you?

A: No.

Q: Do you have any documentation that Jenny makes $30,000 per year gross.

A: No. I was talking to her younger brother a few weeks ago and he told me how well she was
doing and how much she makes.

Q: Did you bring your expenses and proof of income?

A: Yes.

Q: Is there anything else that you want the court to know? Do you have any other evidence to
support your claims with you.

A: Just that my expenses are higher since Joey was born and that my wife has been unemployed
and cares for our baby. Also, that our income has drastically decreased. Our expenses are more
than our income now and we could really use a break. I do not have any other documents.
