Glencrest Dr. and Edmore Pl.), Petaluma Dr. (north of Roscoe Blvd.), Petaluma Pl., Roscoe Blvd. (east of Echelon Pl.),

210 Fwy Eastbound On-Ramp, Dale Av., Green Verdugo Dr., Ranch Caballo Dr. (at Sunland Blvd.), Shadow Way,
Shadow Island Dr., Sunland Blvd. (between Sunland Pl. and 210 Fwy), Teazle Canyon Rd., and
Warnom Av. (south/above Sunland Blvd.), and trails below burned canyons and hillsides. Also included are residences
that have been advised to be prepared to evacuate with forecasts of any or significant rain.

Sunland - Tujunga Area


PHASE 2:
Anticipate moderate mudflow/sediment deposition in Haines Canyon Channel (between Valaho Dr. and
Mount Gleason Av.); and on streets and canyons, below the burned hillsides: Chapin Ln., Chapin Wy,

Placerita North Fork and Canyon Country Area


PHASE 1:
Anticipate minor to moderate mudflow/sediment deposition on Placerita Canyon Rd. and trails below the burned
hillsides and in Placerita Creek in the burned canyons.

Sand Canyon Area


PHASE 1:
Anticipate minor to moderate mudflow/sediment deposition on Pineview Rd., Cambria Estates Ln., Falter St., Iron
Cyn Rd., North Iron Cyn Rd., Pacy St. located south of Placerita Cyn Rd, Placerita Cyn Rd., Sand Cyn Trl., Sand Cyn
Rd., Silver Star Ln., and Warm Springs Dr.. Also anticipate potential mudflow/sediment deposition at culvert/dip
crossing locations below the burned area on Placerita Cyn Rd. and dip crossing locations within Iron Cyn channel.
Anticipate potential minor to moderate mudflow affecting residences on Brooken Ave., Iron Cyn Rd., North Iron Cyn
Rd., Josel Dr., Pacy St., Placerita Cyn Rd., Ranch Creek Rd., Silver Star Ln., and Warm Springs Dr..

Bear Canyon and Oak Springs Area

Canyon Rd., Santa Clara River Streambed, and other trails below burned canyons and hillsides.

FISH FIRE

City of Duarte Area


PHASE 1:
Anticipate minor to moderate mudflow/sediment deposition on streets below the burned hillsides: Greenbank Av. from
its northern terminus to about 850 feet south of the street including intersection area with Brookridge Rd., Melcanyon
Rd. from its northern terminus to about 450 feet south of the street, Brookridge Rd. from Melcanyon Rd. to Opal Canyon
Rd., and Mountain Crest Rd. from its northern terminus to Shadylawn Dr.. Also anticipate minor to moderate
mudflow/sediment deposition in areas below the burned hillsides: adjacent area west of Cedarwood Av near its northern
terminus., downstream area near Las Lomas Debris Basin, area west of Greenbank Av near its northern terminus.,
northern foothill area between Greenbank Av. and Bettyhill Av., Glenn Miller Park of City of Duarte including the area
above Valley View Elementary School, northern foothill area above Brookridge Rd. to the east of Mountaincrest Rd.

City of Azusa Area


PHASE 1:
Anticipate minor to moderate mudflow/sediment deposition on streets and canyons below the burned hillsides: Van
Tassel Motorway, Fish Canyon Rd., San Gabriel River Streambed, and other trails below burned canyons and hillsides.

OLD FIRE
PHASE 1:
Anticipate minor to moderate mudflow/sediment deposition on streets and canyons below the burned hillsides:
Mulholland Hwy, Dry Canyon Cold Creek Rd., Old Topanga Canyon Rd., Hill Rd., Dry Creek Canyon Park area, and
access road in Calabasas Creekside Park & Community Center including baseball field.

CALGROVE FIRE
PHASE 1
Potential mudflows occurring in burned canyons above Crescent Valley Mobile Home Community, The Old Rd., Line
E Lower Debris Retaining Inlet (DRI) on east of Creekside Dr., Newhall-Hidden Valley tennis court, Calgrove Blvd. east
of Creekside Dr., Agramonte Dr., Haskell Vista Ln., Cross St. (Wildwood Canyon Open Space), and Wildwood Canyon
Road. It is recommended that emergency response entities monitor the following areas for potential minor to moderate
flooding and sediment deposition: Drive way within the Crescent Valley Mobile Home Community; The Old Rd. from
Edison Rd. to 2,700 feet west; Access road to a water tank south of Briardale Way from 300 feet to 100 feet south of
its cul-de-sac.; Calgrove Blvd. from 500 feet to 700 feet east of Creekside Dr.; Haskell Vista Ln. all portions including
an intersection with Cross St.; and Wildwood Canyon Rd. from terminus of its cul-de-sac to 3700 feet north. Also
included are residences that have been advised to be prepared to evacuate with forecasts of any or significant rain.

GENERAL NOTES

The debris and mudflow potential forecast is based upon a review of watershed conditions and
weather forecasts from the National Weather Service and other weather predicting sources. It
represents conditions within the watershed that could lead to debris and mudflow events. Debris
and mudflows are highly unpredictable events. The following description is to assist storm
response Divisions and emergency response agencies to plan accordingly:

No Phase Rain is forecast by the National Weather Service, but the amount of rain and/or
watershed conditions are not anticipated to produce appreciable amounts of debris
on streets or specific public infrastructure locations. Reports of debris on private
property may be received by emergency response entities. Few, if any, structures
are anticipated to be endangered, other than those advised to be prepared to
evacuate with any forecast of rain.

Phase 1 Small isolated debris and mudflows possible at specific public infrastructure
locations. Streets may be flooded or blocked by debris. Reports of debris on
private property may be received by emergency response entities. A few structures
may be endangered, in addition to those advised to be prepared to evacuate with
any forecast of rain.

Phase 2 Moderate debris and mudflows possible at more widespread locations. Some
streets may be completely blocked by debris. Depending on location and terrain,
some structures may be endangered, in addition to those advised to be prepared
to evacuate with any forecast of rain.

Phase 3 The potential exists for significant debris and mudflows to be widespread over
specific areas. Streets may be blocked and considered unsafe for travel. Many
structures could be endangered by debris and mudflows. Extreme caution should
be exercised below uncontrolled canyons due to sudden debris flows.
