requiring intravenous anti-biotic therapy. He is
scheduled to receive his next dose at 10 a.am.
The medication delivered by the pharmacy is
labeled with the correct drug and dose, but
with another patient’s name.

At the station….

Nurse: Ohh.. let me see who’s patient ha a

medication for 10 a.m today(looking at the
charts)Mr. Francois Baptiste has an intravenous
anti-biotic therapy which is ceftriaxone 1g every
12 hours TIV .

Nurse opens the drawer and saw that the vial
