WITHIN THREE (3) DAYS after service of this notice on you, you are required to pay
said rent in full, or to quit and deliver up possession of said premises to the undersigned
or their authorized agent, or legal proceedings will be commenced against you to recover
possession of said premises, to declare said agreement forfeited and to recover damages
for the unlawful detention of said premises.

YOU ARE FURTHER NOTIFIED, that the Lessor does hereby elect to declare a
forfeiture of the agreement under which you hold possession of the premises described
above.

If you fail to comply with the terms of this notice, legal proceedings will be instituted
against you to declare the forfeiture of any lease or rental agreement whether written, oral
or implied, under which you occupy the premises described above, and to recover
possession of said premises, to recover court cost, attorney’s fees as permitted by law,
and you may be liable for additional statutory damages of up to SIX HUNDRED
DOLLARS ($600.00) in accordance with California Code of Civil Procedure § 1174(b),
as a result of your failure to comply with the terms of this notice.
