Care Pathways 2011 is the two day annualinternational conference for all thoseinvolved in monitoring and improvingpatient care through Care Pathways,clinical pathways and critical pathways.

ClinicalPrograms and Quality Initiatives Cancer Care OntarioCanada will discuss using a disease managementpathway approach to identify gaps in quality processand the patient experience; driving and performancemanaging regional disease site quality improvementsefforts and supporting policy advice and decisionsthrough modelling. Day one continues with a varietyof presentations, workshops and focus sessionsincluding; developing andusing Care Pathways to meet thequality and productivity challenge; what arethe critical success factors for a successful CarePathway; implementing a pathway to improve theclinical management of a high risk condition;developing a generics pathway that links to conditionspecific pathways; delivering cross boundary andpartnership patient pathways and developingpathways across health and social care.Day two opens with a focuson
‘Involving Patients inthe development and use of Care Pathways'
andwill discuss a practical approach to patientinvolvement in care pathways.
