MAKI MADNESSRA Sushi Official Sushi-Eating Contest Entry Form & Rules
(Note: Contestants must be at least 18 years old to participate.)
RULES & CONDITIONS:#1.
This is a single elimination contest. The first, second, third and fourth place winners will be determined onSunday, March 20 during the Championship Sushi Showdown round at approximately 6:30 pm at RA Tustin,The District, 2401 Park Avenue, Tustin, CA 92782.
#2.
Organized like a bracket system, MAKI MADNESS will take place in heats. Contestants are competingagainst each other. During the first three rounds, the competitors who eat two uncut RA Sushi signatureTootsy Maki the fastest, advance to the next round.

To begin, there will be a qualifying round at 3:00 pm to eliminate any overflow of registrations (upto 8 competitors)
and fill the last spot of the bracket
. Registrations will be accepted on a firstcome-first served basis. When the bracket is filled to capacity, RA Sushi will create and maintain awaitlist. Those interested in being placed on the waitlist must complete a registration form. Theorder of the waitlist will be determined based on when persons’ registration forms are received byRA Sushi. If a registrant fails to appear to compete in a round no later than five (5) minutes after the designated start time of that round (time being kept by RA Sushi in its sole discretion), he/sheforfeits their spot in the bracket to the individual who is at the top of the waitlist (provided thatsuch individual at the top of the waitlist is present at the appropriate time). Any participant drawnfrom the waitlist will compete in the current round and any future rounds of the Contest as ifhe/she had progressed through the bracket system from the beginning (i.e., without being on thewaitlist). In agreeing to be placed on the waitlist, a person understands and agrees that he/shemay not have an opportunity to participate in the Contest and RA Sushi shall have no liability tosuch person if he/she does not participate in the Contest. Waitlist will be maintained and updatedby RA Sushi in its sole discretion. Those who register (regardless of whether they remain on thewaitlist or actually do participate in the Contest) will receive a RA Sushi sweatband andcomplimentary appetizer card (total approx. retail value: $15 each), while supplies last.Employees of RA Sushi, its parent company, subsidiaries and affiliates are not eligible.

The first round for MAKI MADNESS will begin at 3:15 pm and is comprised of four heats (each firstround heat has maximum of 10 competitors) eliminating half the competitors per period withheats taking place every 15 minutes.

The second round is comprised of two heats, with two groups from the bracket competingsimultaneously per heat. (Each group will have a maximum of 4 competitors, each heat with amaximum of 8 competitors) eliminating half of the players per group.

:
The two groups arenot competing against each other; they are just competing at the same time. Heats will takeplace every 15 minutes beginning a half hour after the conclusion of the first round.

The third round is comprised of two heats (each with a maximum of 4 competitors from each sideof the bracket) eliminating half of the competitors with heats taking place every 15 minutesbeginning a half hour after the conclusion of the second round.

The top two from each heat will advance to the Championship Sushi Showdown. During theChampionship Round, the four remaining competitors will eat as many Tootsy Maki as they can inexactly three (3) minutes with only one glass of water.

Soy sauce, wasabi, ginger, hot sauce, or any other types of dressing/sauces/condiments/garnishes willnot be permitted. Only RA Sushi will prepare the sushi for this Contest and the sushi must be consumedexactly as prepared by RA Sushi.

In the event of a tie for First Place (i.e., the Grand Prize), an “eat-off” will then occur. The “eat-off”cannot begin until 10 minutes after the end of the Championship Round; in the “eat-off,” the tiedcompetitors will eat sushi for a one-minute period. Whoever successfully consumes the most sushi will bedesignated as the Grand Prize winner. Additional “eat-offs” will occur should a tie remain.

RA Sushi is not responsible for any errors that may occur in conjunction with administration of theContest, which may include inaccurate counts of sushi eaten by a competitor or mistakes as to Contesttiming. One cup of water will be provided to each contestant prior to the start of each heat. Should anycontestant leave the sushi-eating area for any reason during the heat, he or she may be disqualified in thesole discretion of RA Sushi. Contestants must completely eat (i.e., chewing and swallowing) a piece of sushifor it to be counted as being eaten. Varieties of sushi include: crab mix, shrimp and cucumber topped withtempura bits. A complete list of varieties of sushi that may be used in the Contest can be obtained uponrequest. IT IS STRONGLY RECOMMENDED THAT PERSONS CONSULT THIS LIST BEFORE DECIDING WHETHER TOPARTICIPATE SO AS TO DETERMINE WHETHER THEY ARE ALLERGIC TO THE SUSHI THAT MAY BE USED IN THECONTEST. The decision of the contest judge(s) is FINAL AND BINDING IN ALL RESPECTS. There are no appeals.

By signing this entry form, you affirm and certify that, to the best of your knowledge and belief, you arephysically able to participate in the contest and that there is no physical, psychological, medical or other reason (i.e., heart condition, diabetes, allergies) why the rapid consumption of a large quantity of sushiwould adversely affect your health and that you understand and assume all risks associated withparticipating in this contest. Without limiting the foregoing, you expressly understand and acknowledgethat the rapid consumption of a large quantity of food could cause vomiting, choking and possibly evendeath. You also agree that emergency care may be provided if deemed appropriate by management atthe restaurant in its sole discretion and that RA Sushi and/or the persons providing such emergency careshall not have any liability in conjunction with the provision of same. In addition, you understand and agreethat no life, accident or other insurance of any kind has been obtained with regard to your participation inthe Contest. IT IS STRONGLY RECOMMENDED THAT YOU CONSULT WITH A PHYSICIAN OR OTHER HEALTH CAREPROFESSIONAL BEFORE DECIDING TO PARTICIPATE IN THE CONTEST.

All contestants must follow the judges’ instructions at all times and behave at all times in a mature, civiland socially acceptable manner which is respectful of the rights of others. Without limiting the foregoing,violating the instructions of judges, any attempt to defraud RA Sushi, heckling or arguing with other competitors or the judges, touching other competitors’ sushi or otherwise interfering with their participationin the Contest or fighting is prohibited and will result in disqualification. And, in such circumstances,disqualification shall not represent the sole remedy available to RA Sushi. If a contestant purges or vomits atany point during the Contest, he or she will be disqualified. Showing up inebriated, with a weapon or pet isprohibited and such contestants will be disqualified. Note that storage facilities for personal items will not beprovided at the restaurant.
#8.
If the Contest is not capable being conducted as originally planned by RA Sushi for any reason (asdetermined by RA Sushi in its sole discretion), RA Sushi reserves the right to modify the Contest and makeprizes available via a method which RA Sushi in its sole discretion deems to be fair, appropriate andconsistent with these Rules. Notice of such action by RA Sushi will be posted at the restaurant.
#9.
RA Sushi reserves the right, in its sole discretion, to record the Contest and activities related thereto (inaudio, visual, electronic or other format) for use for publicity, trade and/or advertising purposes. Byparticipating in the Contest, you grant to RA Sushi the right to use your name, nickname, likeness,photograph, image and other indicia of persona for publicity/trade/advertising purposes to the fullestextent permitted by law, without compensation to you or your pre-approval. You further agree to confirmsuch grant of rights in writing upon RA Sushi’s request, without compensation.

PRIZE RESTRICTIONS:
No prize substitution. No prize transfer. Gift certificates are subject to terms andconditions indicated thereon.
