MIKE OUZAS – LAS VEGAS GOALIE TRAINING

3-Day Summer Camp in June 2011

Goaltending is the most important position in hockey, yet it is also the most neglected
position by teams and coaches in any organization. My goal is to provide the city of Las
Vegas with a camp dedicated to teach goaltenders of all ages the many aspects of
goaltending. With over 10 years of goalie coaching experience, I look to bring a high
level of instruction and a carefully designed program to improve abilities in goaltenders
of all ages.

This program will cover everything from basic drills to advanced scenarios, practicing
the many situations a goaltender may see during a game. Each day will also include an
off-ice session, where the students will learn proper warm-up dynamics along with
speed, agility and reaction drills. In addition to the on-ice and off-ice training, video
sessions will be presented to demonstrate the on-ice scenarios of the day.

GOALTENDING CAMP

Each day includes: On-Ice Session (1.5 Hours)

Off-Ice Training (1 Hour)

Group 1: 8:00am-9:30am, Group 2: 9:30am-11:00am

PARENTS/COACHING SEMINARS

Two seminars will be offered in addition to the goaltending camps. Each seminar will
cover different aspects of goalie coaching. My goal is to teach parents and coaches
about goaltender training and psychology. The seminars will run approximately 1 hour
each, and will follow the Saturday morning session of the camp:

Waiver and Release of all Claims

Please read this form carefully. When you sign this form, you waive and release
all claims for injuries encountered from training at Mike Ouzas – Las Vegas
Goalie Training.

Acknowledge Risk Injury: As a participant of Mike Ouzas – Las Vegas Goalie

Training, I recognize and acknowledge that there are certain risks of physical injury in
hockey and off-ice training, and I agree to assume the full risk of any injuries, damages,
loss, including death, and/or other related accidents medical/non-medical which the
camp attendee may sustain as a result of participation or use of the arena, off-ice
activities and/or on-ice programs.

Personal Player Insurance: I understand as a player in USA Hockey that my child

and/or I am currently covered by USA Hockey Insurance, and my child/my policy is up
to date and current.

Waive, Release & Indemnify: I hereby waive, release and discharge any and all claims
I may have or may acquire against Mike Ouzas, Mike Ouzas – Las Vegas Goalie
Training, its instructors, shooters, employees, the staff of Fiesta Rancho Arena, and the
management/staff of all Las Vegas hockey organizations, as a result of mine or my
child's participation in the activities and programs of Mike Ouzas – Las Vegas Goalie
Training. I also agree to indemnify and hold harmless Mike Ouzas, Mike Ouzas – Las
Vegas Goalie Training, its instructors, shooters, employees, the staff of Fiesta Rancho
arena, and the management/staff of all Las Vegas hockey organizations, from any and
all claims resulting from injuries, damages, and losses, including death, and/or other
related accidents medical/non-medical while I or my child are in the Fiesta Rancho
Arena. If a serious injury occurs, this waiver also authorizes a representative of Mike
Ouzas – Las Vegas Goalie Training or a qualified driver to transport the camp
participant to receive medical treatment.
