Conusion aterhospital stay
Anyone who’s been in the hos-pital can remember the eelingo unreality that comes rom thenoises and sleep interruptions inthat environment, not to men-tion eeling unwell. And goinghome doesn’t automatically makeeverything ine again: Many se-niors experience loss o memory, conusion and cognitive problemswhen they’re discharged rom thehospital.Te National Institute o Agingcommissioned a study on this phe-nomenon, and it was completedby the Northwestern University Feinberg School o Medicine.wo hundred seniors age 70-plus who had no previous cognitiveor dementia issues were given aMini-Mental Status Examinationupon release rom the hospital.Tis 30-question test only takesa ew minutes to administer, andchecks or things such as orienta-tion to time and place, calculation, attention and recall. Te patient isasked to repeat three words, telltime, know the day o the week, spell a word backward and copy agure with pencil and paper.Nearly one-third o those se-nior patients being released romthe hospital had low cognitionscores. his means that seniorscoming out o the hospital mightnot understand the instructionsthey’re given or medications andsel-care.ested again one month later, their scores had risen considerably or things like reading, writing, calculations and comprehension. One conclusion o the study wasthat “patient sel-management”training -- teaching the patientabout his or her medications andcare -- should be handled laterrather than at the time the patientis being sent home. Another con-clusion was that doctors and amily (and I’d add “riends”) need to stepup, be aware o the phenomenonand give assistance -- or at leastarrange or it.Matilda Charles regrets that shecannot personally answer readerquestions, but will incorporatethem into her column wheneverpossible.
