thosefactsarehereinprovided.AlltimesprovidedareinAlaskaStandardTime.ProgresswithrespecttotheorderedactionsshallbereportedonamonthlybasistoCGDSEVENTEEN(dcs)untilallorderedactionshavebeenimplemented.2.Thisinvestigationwasdirectedforthepurpose
of
ascertainingthefactsandcircumstancesassociatedwiththemishapandtoproviderecommendationsfortheimplementation
of
improvedprocedureswhich,ultimately,reducetherisk
of
similarincidentsoccurringinthefuture.Theactions
of
themasterandcrew
of
theTugPATHFINDER("PATHFINDER")havebeeninvestigatedunderaseparateCoastGuardMarineCasualtyInvestigation.Thisinformaladministrativeinvestigationwasnotintendedtodeterminethecause
of
thePATHFINDERgrounding.Thisinvestigationmustbeviewed
in
conjunctionwiththeMarineCasualtyInvestigationtofullyunderstandthecause
of
thiscasualty.3.FactualSummary.

December2009,theShipEscort/ResponseVesselSystem(SERVS)directedthePATHFINDERtoserveasanIceScoutVessel(ISV)toconducticescoutingoperationsand

designedasaportspecificguidetofurtheraidandenhancethesafety
of
tankernavigationandtheprotection
of
themarineenvironmentwithinthewaters
of
thePrinceWilliamSound.ThePrinceWilliamSoundRegionalCitizensAdvisoryCouncil(PWSRCAC),theSERVS,andtheCoastGuardCaptain
of
thePortValdezareallsignatories
to
theVERP.

TheVERPrequiresan"icescout"tovisuallycheckforiceinthevicinity
of
BlighReefnolongerthansixhourspriortoatankertransitingPrinceWilliamSound.Anicereportwasrequiredduetothescheduleddeparture
of
thetankerKODIAKwhichwasscheduledtodepartoutboundfromthePort
of
Valdezlaterthatevening.AlthoughthePATHFINDERwasdesignated
as
anISVfor
23
December2009,anicescout
is
notspecificallyrequired
to
beatug.Virtuallyanyvessel
or
evenanaircraftcanserve
as
anicescout.Theprimaryobjective
is
tovisuallyidentifyanyfloatingicethatmayendangeratankertransitthroughtheTSS.

AnISV
is
defined
as
"anysuitablysizedvesselequippedwithanoperationalradarandsearchlight."Duetosizeandpowerlimitations,thePATHFINDERdoesnotprovidetankerescortservicesandwasregularlychosenbySERVS
to
perfolmicescoutingduties.

Atapproximately1402,thePATHFINDERcheckedintothePrinceWilliamSoundVesselTrafficServiceArea(PWSVTSA)andindicatedtheywereenroutetoBlighReef
to
providetheicereport.AsaVesselMovementReportingSystem
(V
MRS)user,thePATHFINDERwasrequired
to
check-inwiththeVesselTrafficService(VTS)toprovideaninitialsailplan,reportanydeviationsfromtheinitialsailplan,orcheck-out
of
theVMRSwhentransit
or
operationswerecomplete.

ThePATHFINDERwasnotrequired
to
check-in
at
any
of
thedesignatedVTSRepOltingPointsinthePWSVTSAbecausethePortsandWaterwaysSafetySystem(PAWSS)atVesselTrafficCenter(VTC)PrinceWilliamSoundreceivedAutomaticIdentificationSystem(AIS)informationfromthePATHFINDER.VTSPrinceWilliamSoundwasalsoabletotrackvesselsoperatinginthePWSVTSA,includingthePATHFINDER,basedoninformationprovidedfromtheUSCGradarsiteonReefIsland.

thistime,the
PATHFINDER
alsostatedthattheywouldremaininthevicinity
of
Bligh
Reef
until1800inordertoprovidean1800icereport.Thiswasinanticipation
of
anoutboundtransitbytheTanker
KODIAK
fromthePort
of
Valdezlaterthatevening.

Earlierintheday,at0520,
Cape
HinchinbrookEntrancewasclosedtoalloutboundladentankervesselsduetoweather.The1800icereportwouldextendthewindow
of
opportunityforthe
TankerKODIAK
todepartValdeztomidnight
if
the
Cape
Hinchenbrookentrancewasopenedlaterintheday.

PATHFINDER
begantocrosstheTSStowardtheinboundlane.

PATHFINDER's
operations.
