"Shootingrangefacility"meansapublicorprivateshooti_ngrangeandthepremisesonwhichtheshootingrangeislocatedandincludesallthebuildings,structures,parkingareas,andotherassociatedimprovementslocatedonthepremises.A"shootingrangefacility"includesanyshootingrangefacilityoperatedormanagedbymembersofaprivateclubororganizationforthebenefitofitsmembers."Superintendent"meansthesuperintendentofpolice.

4-151-020License-Required.

Itshallbeunlawfulforanypersontoconduct,maintain,oroperateashootingrangewithoutfirstsecuringashootingrangefacilitylicenseunderthischapter.Thelicenserequiredbythischaptershallbeinadditiontoanyotherlicenserequiredbylaw.Thelicenseshallbevalidonlyforthepremisesforwhichitwasoriginallyissuedandonlytothepersontowhomitwasissued.Thelicenseshallnotbesubjecttosale,assignment,ortransfer,voluntaryorinvoluntary.Thelicenseshallbedisplayedinaconspicuousplaceintheshootingrangefacility.

(a)AnapplicationforashootingrangefacilitylicenseshallbemadeinconformitywiththeprovisionsofthischapterandthegeneralrequirementsofChapter4-4relatingtoappicatlonsforlicenses.Nooriginalorrenewallicenseshallbeissuedtoanyapplicantorlicenseeunlessallpersonsrequiredtobedisclosedasapplicantsmeettheeligibilityrequirements.(b)Theapplicationshallbeinwriting,signedbytheapplicantifanindividual.Iftheapplicantisapartnershiporcorporation,theapplicationshallbesignedbyanofficerorpartner.Iftheapplicantisalimitedliabilitycompanymanagedbymanagers,theapplicationshallbesignedbyamanager.Iftheapplicantisalimitedliabilitycompanymanagedbyitsmembers,theapplicationshallbesignedbyamember.Theapplicationshallbeverifiedbyoathoraffidavit,andshallincludethefollowingstatementsandinformation:(1)inthecaseofanindividual:thename,dateofbirth,residenceaddress-currenttelephonenumberandsocialsecuritynumberoftheapplicant;inthecaseofapartnership,limitedpartnership,corporation,limitedliabilitycompanyorotherlegalentity:thedateofitsorganizationorincorporation;theobjectsforwhichitwasorganizedorincorporated;thename,residenceaddress,dateofbirthandsocialsecuritynumbersofanypersonowningdirectlyorbeneficiallyanypercentageofownershiptherein,provided,however,thatifthepartnership,limitedpartnership,corporation,limitedliabir~ycompanyorotherlegalentityispubliclytradedonanexchangewithinthemeaningoftheSecuritiesExchangeActof1934,thenames,residenceaddresses,socialsecuritynumbers,datesofbirthandpercentageofinterestofthethreememberswhoownthehighestpercentageofinterestthereinandofanyothermemberswhoholdafivepercentorg~aterinteresttherein;and,whereapplicable,thenames,residenceaddresses,datesofbirthandsocialsecuritynumbersofallprincipalofficersanddirectors;iftheentityisamanager-managedlimitedliabilitycompany,thenames,residenceaddresses,datesofbirthandsocialsecuritynumbersofallmanagers;andthenameandcurrenttelephonenumberOfanyauthorizedagent;andinallcases:thename,address,andabriefdescriptionofanyworkperformedbyanypersoninconnectionwiththepreparationandfilingofthe.application,includingbutnotlimitedto,anyattorney,accountant,consultant,expediter,promoterorlobbyist;(2)thecharacterofthebusinessoftheapplicantandthelengthoftimetheapplicanthasbeeninabusinessofthatcharacter,orinthecaseofacorporation,thedateofincorporation;(3)thelocationanddescriptionofthepremisesorplaceofbusinessw~ichistobeoperatedundersuchlicense;(4)thenameandaddressoftheownerofthepremises.Ifthepremisesareleased:(A)acopyofthelease,whichmustincludeastatementofthebuildingownerindicatingthattheowneragreestotheuseofthebuildingfortheoperationofashootingrangefacility;(8)thename,addressandtelephonenumberoftheowner

of

thepremises,includingthenameandaddressofthebeneficiaryiftitletothepremisesisheldbyapersonastrusteeandifknowntotheapplicant;(5)thenameandidentityofallindividualswhowillbemanagersoftheshootingrangefacilityand

a

copyofeveryagreementforthemanagementoftheshootingrangefacility;(6)theapplicant's,manager'sandemployees'federalfirearmslicense,CFPandFOlDcardnumbers;-3-
