ASSESSMENT

SUBJECTIVE DATA:

DIAGNOSIS Knowledge Deficit related to unfamiliarity with information resources INFERENCE: Absence or deficiency of specific cognitive information necessary for the patient to make informed choices regarding condition, treatment and/or lifestyle changes)

PLANNING After 1 hour of the desired nursing intervention, the patient will be able to verbalized understanding of the condition, disease process and treatment

INTERVENTION Determine clients ability, readiness and barriers to learning Provide positive reinforcement. Avoid use of negative reinforcement. (eg. criticisms)

RATIONALE Serves as a baseline data

EVALUATION After 1 hour of the desired nursing intervention, the patient verbalized understanding of the condition, disease process and treatment

Hind ko naman alam yung Myoma, kaya kahit matagal na akong nakapagpaUltrasound, di ko pa tin pinapansin dahil walanaman akong nararamdaman! as verbalized by the patient

This wont help the patient towards learning and negative reinforcements may alleviate patients anxiety and confusion about things that she doesnt know. To facilitate learning and recall

Use gestures or facial expressions that help convey meaning of the information Provide written information or guidelines and self learning modules for the patient to refer to as necessary Provide active role for patient learning process

Reinforces learning process, allows the patient to proceed at own pace

Promotes sense of control over situation and it means determining that the client is assimilating and using new information
