It detects early diseases, promotes wellness, monitors compliance to medication and minimizes absenteeism among employees. In view thereof, we would like to ask permission from your good office to allow our department to conduct Employees Annual Screening for 2010.The activity will start August 1,2010 to December 31, 2010. Attached herewith is the Employees Clinic Annual Screening Guidelines for your perusal.

West Visayas Medical Center Department of Family & Community Medicine EMPLOYEES CLINIC ANNUAL SCREENING GUIDELINES 2010 1. All personnel (Permanent, Casual, Regular, One Job) are qualified to avail of the annual screening under the Employees Clinic Program of the Department of Family and Community Medicine (DFCM) starting AugustT 1,2010 to December,2010 at the OPD Clinic of the department. 2. Specific dates will be assigned for each department. Schedules will be posted one week before the actual activity. Chairman or Head of Office will also be notified. If however, an employee could not come on his/her scheduled date, he/she will be accommodated after completing the existing schedule. 3. All laboratories will be FREE of charge. Only laboratory request a bearing the official stamp by department will be honored. Arrangement was already made with Department of Laboratory. 4. Employees without co-morbidity can only avail of routine laboratories such as CBC, Urinalysis, Fecalysis, Chest Xray and ECG. 5. Specific laboratories will be requested to employees with co morbidities. This will be based on the existing Philippine clinical practice guidelines (CPG) if applicable. 6. Abnormal laboratory results will be manage by the residents upon referral to his consultants. If an employee requested management of other attending physician, a copy of laboratory result will be attached to his files at DFCM Employees Clinic for documentation. 7. Employees who have unremarkable laboratory results within the past 3 months may avail of the annual physical examination and maybe requested appropriate laboratories based on the residents clinical findings. 8. A Family Health Care Record and Wellness plan will be made for each employee including his/her family members if feasible. 9. Immunization record of each employee will be update.

10.High risk employee assigned in areas such as isolation room for rabies patients will be given rabies pre exposure prophylaxis (3 doses) depending on availability of the vaccine. Such employee will be notified of his scheduled immunization. *Employees not listed due to technical error may approach the residents of department of Family Medicine for inclusion to the annual screening.

Residents
1. Coordinate retrieval of all other records of the employee and compile for proper documentation at the Employees clinic of the department. 2. Make or update the Family Health Care Record and if feasible make family wellness plan for the employee and his family members. 3. Request necessary laboratories basing on the Employees Annual Screening Guidelines 2010 as approved by the medical center chief. 4. Updates record about immunization received. 5. Elicit antibody status for Hepatitis B. Request Anti HBs antibody if results are questionable or advice repeat immunization for hepatitis B. 6. Advice high risk employee assigned at special areas attending on suspected and patient diagnose with rabies about the availability of anti rabies pre exposure vaccination. 7. Refer difficult cases to consultant n deck. 8. Should be compassionate utilizing the five star quality of a family physician in dealing with the hospital employee or any other patient in particular.
