STATEMENT OF WORK
USPS FITNESS CENTER MANAGEMENT1. Background
The location of the project is at the United States Postal Service Headquarters (USPS HQ)Building, 475 L’Enfant Plaza, SW, Washington, DC 20260. The objective of this project is toprovide a comprehensive health promotion program for USPS HQ employees as described in therequirements section of this document. The program components shall be afforded to allmembers and are demonstrated, in part, through the fitness and health assessments, individualprogramming, lifestyle counseling and educational programs, targeted interventions and facilityupkeep. The components must be administered in a professional manner by the fitness center staff.Approximately, three hundred (300) USPS HQ employees use the fitness facility as activemembers with approximately eighty-five (85) member visits per day. The Fitness Center stafprovides several exercise classes throughout the day as well as monthly lecture(s) on healthand/or nutrition topics. By appointment, employees can obtain dietary information, physicalassessments and personal training.
2. Scope of Work
The Contractor shall provide all necessary labor, equipment and supplies (other than thoseprovided herein), to operate an on-site Fitness Center at USPS Headquarters (L’Enfant Plaza).
3. Requirements
The Contractor shall do the following:
Provide proper supervision of the exercise floor and exercise room
Oversee the cleaning and maintenance of the fitness center (provided by USPS) andReport any cleaning/maintenance deficiencies to the Contracting Officer Representative(COR).
Secure and maintain a preventive maintenance contract for Fitness Center equipment.
Ensure that personnel working under this order have knowledge and skill in operating anAED.
Assure the upkeep of the records and ensure all members records are kept in a securelocation.
Verify membership prior to users using the fitness center. Provide a quarterly report thatshall include the number of participants in each Health Promotion Center for the quarter and an annual report with a cumulative total for the year.
Monitor and maintain fitness equipment at the Fitness Center, in accordance with theManufacturer’s recommendations. NOTE: Maintenance and cleaning of the locker rooms and Fitness center space will be the responsibility of the USPS.
Provide consultation and layout of any new fitness center proposed for USPS employees.
Track the number of participants and request and analyze evaluations of the programs.Provide a Quarterly Report that includes the number of participants, evaluations of theprograms, and recommendations for change or improvement within existing resources.
4. Programs
 
The Contractor shall conduct:a.Onsite Health Promotion ProgramsThese activities shall be in accordance with the American College of SportsMedicine (ACSM) guidelines and other applicable guidelines specified below.The contractor shall perform Worksite Health Promotion activities that consistof the following:A minimum of twenty (20) health promotion programs per year. The programs shouldfocus on stress management, nutrition and weight management, back care andcardiovascular health. The programs may consist of lectures, screenings and/or workshops. The Contractor is expected to arrange for appropriate instructions for theseprograms. They will oversee the development of the program content, lesson plan and beresponsible for all audio-visual aids and/or handouts.b.Health Promotion Courses
A schedule and an outline of any Health Promotion program will beprovided by the Contractor to the COR within five working days prior to thescheduled course. The Quarterly Report shall include the number of participants in each course provided during the quarter and an annual reportwith a cumulative total of participants for each course for the year, and ananalysis of the participants’ evaluation forms by individual courses, thesuccess rates, special problems, that may be used to improve the coursesprovided.
Provide and subject a schedule and an outline of any training to beprovided to the COR within five business days prior to the trainingcommencing.
Provide a Quarterly Report with the following information: the number of training classes delivered, success rates, special problems encountered,which may be used to improve or change training program components.
Occasionally participate in programs that are identified by the COR(approximately two per year).
Prepare promotional materials for health education programs.c.Additional CoursesExercise Classes: Exercise Classes shall be supervised and shall be offeredat various times. The USPS reserves the right to add or delete or change thetimes for aerobic classes at any time during the life of this contract. TheContractor may be asked to provide specialty non-aerobic exercise classes,(i.e. Yoga, T’ai Chi, etc.), (approximately 3-4 classes offered every week – itis expected that the number of exercise classes offered shall increase inaccordance with the increase in usage), and stress management(approximately one course per quarter). Course attendance requirements willbe proposed and agreed upon prior to offering. At least one class per quarter shall address stress management. (Historically there were approximately 208general exercise classes conducted over the past year, to include, but are
 
not limited to high impact aerobics, low impact aerobics, and stretchingexercise classes).d.
 
Postal Stampede
The Contractor is responsible for organizing all aspects of the PostalStampede, which is a 5K team walk that occurs in the spring to promotephysical activity among USPS HQ employees. The event generally drawsthree hundred (300) participants. Organization will include, but is not limitedto: marketing, recruitment of sponsors, operation of the actual event, andevent set up and clean up.
5. Reporting
The Contractor shall:
1.
Provide quarterly, semi-annual and annual reports to the COR in the established format.The Quarterly Report shall include the following information:Brief narrative of significant events;Worksite health promotion courses (number of participants, analysis of participants’ evaluation forms, success rates, special problems encountered,etc.);Training classes (number, success rates, special problems encountered, etc)which may be used to improve or change training program components;Special Health Promotion Programs (number, success rates, specialproblems encountered, etc).Major accomplishments;Review and evaluation of programs; problems encountered withrecommendations for resolution.
Recommendations for improvement.2.Document and report all accidents or injuries in the fitness center to the COR no latethan one (1) business day after the incident.3.Equipment Recommendations and Documentation – ongoing and documented quarterlyHealth Promotion/Screening Program Plan and ScheduleThe Contractor shall submit a plan for the development of the health promotion/healthawareness program and a schedule for its implementation within fifteen (15) workingdays after contract award and within thirty (30) working days prior to the exercise of anoption.4.Member Satisfaction SurveysEach December and June, a survey will be distributed to all members. The results of these surveys shall be provided to the COR for review thirty (30) days after compilation.
