3. All grades should be indicated in the Midterm, Final, and Final Ratings.
4. No Grading Sheet shall be treated valid without the signature of the Instructor, Dean, VPAA, and the Registrar.
5. No final grades can be changed after submission.
6. All rentries should be readable.
7. Erasures should be properly initialed.
8. Remarks should be individually filled up.
9. Use extra page when necessary.
10. Do not leave blank spaces in the Final Rating.
