Liability: Makeup At Tiffany’s LLC., is acting as a non-exclusive independent contractor for the
client. Each vendor is responsible for their own actions and enforcement of their contract.
Makeup At Tiffany’s LLC will not be responsible for any type of allergy, breakout or discomfort
from makeup application. We pride ourselves in giving nothing but the best.

I understand there is a travel fee of $25 PLUS tolls for locations up to 20 minutes each way, 40
roundtrip, of our location in Newburgh, NY. Any time longer will be negotiated.

Your date is not held until this form is completed, signed and returned to us with the full deposit.
The deposit will be 50% of the total amount. Therefore, if there are 4 makeovers the deposit for
all four must be sent in order to hold the date and guarantee makeup application for all four.
Makeup At Tiffany’s has the right to book other events before or after any event as long as the
original number of makeovers have been completed. The deposit will be NON-REFUNDABLE.

Changes/Cancellations: Change of confirmed wedding or event date is subject to availability of

Makeup At Tiffany’s and must be within one year. Your deposit will not be refunded if you
cancel.
