This evaluation and management coding tool is designed to help health care providers calculate and verify the appropriate coding level for patient encounters. The E and M Coding Tool uses the 1995 and 1997 Guidelines for Evaluation and Management Services and is an adaptation of a Center for Medicare and Medicaid Services audit tool.
