RISK ASSESSMENT
Pulmonary insufficiency
Cardiac insufficiency
Mitral valve insufficiency

Cardiovascular insufficiency

Hypertension
Dehydration
Malnutrition
Diabetes
Hypothyroidism
Bleeding disorder
Allergies to medication
Allergies to anesthesia
 
PREPARATION OF THE PATIENT:
Blood tests
Urine tests
X-ray chest
X-ray of bone
CT scan
MRI
Biopsy of bone
Myelogram
Aspirin was stopped before procedure
Patient was on fasting from ______before surgery
Part was prepared and draped in sterile fashion
ANESTHESIA:
General anesthesia
POSITION OF THE PATIENT
Supine position
THE PROCEDURE
With patient in supine position the part was cleaned withantiseptic solution and draped
Incision was given in the skin over the bone defect
 _____muscles were retracted
The blood vessels adjacent to the bone was preserved
Nerves adjacent to the bone was preserved
Bone was exposed and observed that bone was damaged withmalignant tumour /benign tumour /injury/____level spinal fusion
Defect/pieces of fractured bone was removed
Graft/ replacement material was placed into the defect
 
After placing the graft it was held in place withpins/plates/screws
The muscles were replaced
The fascia and skin was sutured
A splint / cast were applied to prevent movement of the boneswhile healing. 
AFTER PROCEDURE:
Patient was shifted to the intensive care unit
Blood pressure, temperature, pulse rate was monitored
DURATION
