Children’s Garden Parent Teacher Conferences 2008

Dear parents,

You have signed up for a parent teacher conference with

The parent teacher conference offers an opportunity for us to share insights and
questions about your child. The following questionnaire is a tool to help us use our 30
precious minutes together most effectively. During that time, I have specific observations
which I will be sharing with you. However, it will be important to hear from you as well.
Your thoughtful completion and timely return of this questionnaire will assist us both in
our preparations. If you have additional questions beyond those listed, feel free to share
those with me as well.

If you choose to return a hard copy, and you need more space, use the back or attach
more paper. If you choose to complete and return the form electronically, use as much
space you need. Please return the form to me directly or by email by Friday, November
21, 3:00 pm.

kinds of things inadvertently "come out" of your child whether songs, verses, or
stories about school, other noticeable changes since school has started).
