We are writingto inorm you o a matter involving some o your personal health inormation at Baylor Heart and Vascular Center, L.L.P.(“BHVC”), an aliate o Baylor Health Care System, in Dallas, exas.On January 26, 2012, we discovered that an Apple iPad was stolen rom the automobile o a BHVC employee. Te employeepromptly reported the thet to law enorcement authorities and to Baylor. Subsequently, Baylor initiated actions to determine what inormation may have been on the stolen device and which patients may have been aected by the thet. Baylor’s resultinginvestigation revealed that the iPad may have retained inormation viewed on the device, but inormation was not intentionally stored on the device.Te inormation potentially contained on the device included the names, dates o birth, procedures perormed and treatingphysician names on certain BHVC patients. Inormation on local health screening patients, including names, addresses and testresults, such as blood pressure, glucose and cholesterol levels, may also have been on the iPad. Te stolen device did not containany social security numbers or other nancial related inormation. While we have no evidence that your personal inormation has been accessed or misused as a result o this incident, we stillsuggest that you regularly review your medical bills (including your “Explanation o Benets” statements) to check or chargesyou do not recognize. Te attached Reerence Guide also provides details on other steps you may wish to consider, includingrecommendations by the U.S. Federal rade Commission on how to urther protect yoursel against identity thet, andinormation on how to place a raud alert on your credit le, although once again no nancial or credit inormation was on thedevice that was stolen.Please be assured that we are taking steps to re-evaluate the security o portable medical devices, such as iPads, to urther protectour patients’ inormation, including the implementation and enorcement o security measures, such as encryption, to protectany data on these devices.
