New premedication guidelines from the American Heart Association
Many patients with heart problems have been instructed to take an antibiotic premedication priorto dental work. For many years, this antibiotic has been prescribed in order to prevent infectiousendocarditis. Infective endocarditis (IE) is an infection of the heart’svalves or inner lining, which is caused when bacteria enter the blood-stream and travel to the heart. This bacteria, found in the mouth, canbe introduced into the bloodstream during various dental procedures.Therefore, many of our patients with heart conditions have followedthe instruction of their physicians and taken an antibiotic prior to anydental work. Now, however, the American Heart Association haschanged their guidelines as to which patients need to premedicate.The new guidelines focus on the patients at the greatest risk of developing a heart infection.
The following conditions requirepreventive antibiotics before dental work:
s
Artificial heart valves
s
A history of infective endocarditis
s
Serious congenital heart conditions (unrepaired cyanotic congenitalheart disease, including those with shunts or conduits; a completelyrepaired heart defect with prosthetic material or device; a repairedheart defect with residual defect at the site or adjacent to the site of aprosthetic patch or a prosthetic device)
s
A cardiac transplantation that develops a problem in a heart valveIf you have any of these heart conditions, or are not certain if you do, please contact yourphysician or cardiologist to determine whether or not you need to premedicate under the newguidelines.
Patients who previously were required to premedicate, and are not now, include those with:
s
Mitral valve prolapse
s
Rheumatic heart disease
s
Bicuspid valve disease
s
Calcified aortic stenosis
s
Congenital heart conditions, such as ventricular septal defect, atrial septal defect, and hypertrophiccardiomyopathyAgain, we recommend that anyone with any heart condition speak with their physician orcardiologist to clarify the new guidelines and, if necessary, receive a prescription for the antibioticpremedication.

The above information pertains to antibiotic premedication for heart conditions.Patients with
joint replacements
may also need to premedicate for dental work. since April 2007. Her additionto our staff has been a great benefit to us and our patients.She comes to us with quite a bit of dental assistingexperience. Martha received her training to work as anassistant in 1997 with the Red Cross at Camp Pendletonin California. She was there visiting her brother in theMarines and decided to stay for dental assistant training.Martha has worked in a couple other dental offices sincethen and has proven to be very proficient in her work.When she is not working here at the office, she enjoysspending time with her three daughters, ages 12, 16, and18. Martha also enjoys gardening and painting ceramics.She lives in Merrionette Park, Illinois.Martha says that one of the things she likes best aboutdental assisting is being with her patients who have a lotof dental work to be done. Seeing the before and after of big dental cases is very rewarding. Martha also findsdental surgical procedures to be very interesting.
