Introduction
1.·WhilstproceedingtowardstheRVforaboattransfersouthoftheCrowlinIslandsonthemorningof22October2010,HMSASTUTEcontinuedonaheadingof130,towardstheIsleofSkyebridge,for4NMuntilonlyminutesbeforegrounding.Thespeedofthesubmarinewasslowedfrom13ktsto2.5ktsbuttheOOWdidnotgiveanyhelmordersthatwouldhaveturnedthesubmarineawayfromthedangeronship'shead.Thiswasdespitebeinginstructedtoonatleast3occasionsbythePOOWandOOWBelow.Asternrevs(16)wereapplied,aswasaturntostarboard,butthesewerenotasaresultofanappreciationofanyimmediatedangertothesubmarinebytheOOW,rathertomanoeuvrethesubmarineontoafavourablecoursefortheboattransferasinstructedbythe
XO.
Duringtheturntostarboardthesubmarinegroundedbytherudder.Earlyattemptstofreethesubmarinebyusingaheadrevs,MBTblowsandde-ballastingfailedandthesubmarineremainedagrounduntilshortlybeforethenexthighwaterat1730Z.DuringtherecoverysomefurtherdamagewasdonetothesubmarineduringaminorcollisionwiththetowingvesselANGLIANPRINCE.2.TherootcausesofthegroundingwerenonadherencetocorrectproceduresfortheplanningandexecutionofthenavigationcombinedwithasignificantlackofappreciatlonbytheOOWoftheproximityofdanger.However,anumberofadditionalcausalfactorswerepresent,includingsomedeficiencieswithequipment.InconductingtheServiceInquirymuchreliancehasbeenplacedonwrittenandspokenstatementsbecauseofapaucityofrecordedelectronicdata
