Honeywell HomMed Enhances LifeStream Management Suite with Personal Emergency Response System (PERS) for Seamless Patient Care Transitions
SAN JOSE, Calif. EMBARGOED UNTIL April 29, 2012 Honeywell (NYSE: HON) HomMed, a global leader in telehealth and remote patient care, announced today the addition of a mobile telecare service to its product offering a Personal Emergency Response System (PERS) from MobileHelp that allows users to summon emergency help anytime, anywhere, with the touch of a button. The PERS integrates with Honeywells LifeStream Management Suite and improves the safety and security of patient care transitions. A first for the telehealth industry, Honeywells LifeStream Management Suite now integrates telehealth and telecare patient data on one dashboard. Designed to provide a broader picture of patients health as they move through care transitions, healthcare providers will have access to both remote monitoring data as well as emergency PERS alerts in one patient record. Honeywells new PERS, in conjunction with the LifeStream Management Suite, effectively allows healthcare providers, patients and families to achieve a new peace of mind around the patients transition to independent living as they move from hospital to home," said Terry Duesterhoeft, president of Honeywell HomMed. The MobileHelp PERS offers enhanced user flexibility by providing the choice of a mobile unit, a base station (landline) unit, or both, as well as a pendant or wrist button. When the patient is at home, it works like a traditional medical alert system. Away from home, the small, hand-held unit can be carried or clipped to a belt, allowing patients the freedom to be completely mobile. A push of the button automatically connects the user to an emergency operator, who can send help to the users exact location. Data from the PERS unit is then integrated back into the LifeStream Management Suite as an alert in the patient record, providing an additional data point for care providers who are monitoring a patients health. The addition of PERS to Honeywells service offering demonstrates our ongoing commitment to the expansion of the LifeStream Management Suite to provide a platform for an ever-widening array of products and services," said Mr. Duesterhoeft. According to the Center for Medicare and Medicaid Services (CMS), nearly one in five patients discharged from the hospital is readmitted within 30 days, at a cost of over $26 billion per year. And according to the independent, not-forprofit organization, the Joint Commission, hospital readmissions are mainly a result of gaps in follow-up care. To that end, Honeywells LifeStream Management Suite the software platform providing the behind-the-scenes technology support for Honeywells devices creates a technology-based constant care for patients as they move between care environments. "Our LifeStream Management Suite spans the healthcare provider gap that exists when patients leave the hospital and transition back into their homes, said Mr. Duesterhoeft. By providing the technology resources to keep patients in constant contact with their care givers, patient issues can be addressed as they arise, outside of the hospital, reducing the rates by which patients need to be readmitted."

LifeStream Management Suite serves as the critical connection point between a patient using a remote monitoring device and their healthcare provider, while also streamlining and organizing patient data as it flows into the healthcare providers system, improving workflow efficiencies among healthcare providers. Honeywells LifeStream Management Suite includes five software applications: LifeStream Manager: As Honeywells leading healthcare-provider facing application, LifeStream Manager interoperably manages all in-home patient monitors from a single dashboard, allowing one healthcare provider to effectively monitor up to 200 patients data at any one time. LifeStream Manager also allows healthcare providers to customize which biometrics are collected from patients and when, increasing overall patient compliance to levels as high as 90 percent. Other features include the ability to view indicator flags when realtime patient data falls out of acceptable ranges, immediately alerting healthcare providers to patients whose data points indicate triage intervention is necessary. LifeStream View: Providing secure access to patient information via the Internet, LifeStream View ensures patients, families and healthcare providers have the ability to access a current picture of the patients health status at any time anywhere they have an internet connection. It also allows patient data points to be organized into tabs and graphs that can be used to help identify trends in the patients overall health. LifeStream Connect: With the widest selection of interfaces to standard electronic health records (EHR), electronic medical records (EMR) and other point-of-care applications, the LifeStream Connects HL7 interface ensures efficient exchange of clinical data in a single platform, eliminating data entry redundancy and errors. Using LifeStream Connect, healthcare providers can use a unified application for patient monitoring and no longer have to switch between disparate systems, ensuring a consistently accurate view of patient health. LifeStream Analytics: Offering powerful analytical tools for healthcare providers to better manage work flows, LifeStream Analytics generates reports that provide the capability to track patient outcomes and patient load for each care provider, as well as hospital admissions and readmissions based on patient diagnosis. It also provides reports that allow healthcare providers to improve patient care by assessing patient profiling trends, device status and utilization, and asset management. LifeStream Benchmarking: Currently in Beta testing, LifeStream Benchmarking offers healthcare providers the ability to track patient outcomes by population, aggregating data from across a pool of registered users and patient data points.
