A NOTE FROM THE HEALTH OFFICE

ATTENTION ALL PARENTS OF 5TH GRADERS

Just a reminder that all incoming 6th graders must have proof of vaccination with Tdap, and Meningococcal Vaccine (MV or MCV), by the first day of the 20122013 School Year. If your child has already received these vaccinations, please bring an updated vaccination record to the Health Office so we may update your students Health Record. If you are planning on getting the vaccinations over the summer, make sure you get the documentation to the Health Office at the Middle School on or before the first day of school in August. We will be sending out notices to the parents of those 5th graders that do not have a record of the required vaccinations or a valid exemption form, at the end of this school year.
