<Use this form when you are using customer interviews to evaluate candidate vendors. This form isdesigned for evaluation of vendors for implementation of a COTS (Commercial Off The Shelf)application. Modify the questions to fit your need.
Create up to three vendor evaluation teams (e.g. composed of Project Team member(s),Technical staff member(s) or User(s)).
Have the teams contact each of the vendor’s customers.
Each evaluation teams should use a separate copy of this form to rate each vendor.
Enter data from Part II of this form into the Customer-Reference Rating Summary worksheet. >
