Children’s Ministry Assessment Form
The First Impression
Is your children’s ministry visible to the first time visitor?Are there accurate signs directing the first time visitor to the children’s department?Is there a welcome center that offers relevant information about the children’s ministry?Do you have a brochure available for first time visitors?The brochure should include the vision, mission and values, safety policy, contactinformation, welcome letter and children’s department programs offered with their meeting times.Is there a welcome and registration process that is efficient for first time visitors?
The Drop off 
Are the hallways in the children’s ministry clear of clutter and sufficient for easy accessto class rooms?Is the appearance of your children’s department: Sanitary?Organized?Age appropriate?Relevant?Attractive?Can first time visitors easily locate their child’s classroom (age specific signs visible)?Is there an efficient and secure check in policy and procedure for each class?Is the check in policy and procedure being followed properly by the volunteers?Are there at least two volunteers per classroom to ensure safety?Are the volunteers in dress code and presenting themselves with cleanliness?Are the volunteers energetic and welcoming to the parents and guardians?Are volunteers engaged in age appropriate activities with the kids?Are the classrooms: Sanitary? Organized? Age appropriate? Attractive?Safe?
 
Ministering to the Kids
Is there age appropriate curriculum in each classroom?Are there age appropriate activities in each classroom?Are the volunteers trained and qualified to meet the needs of the children they areministering to?Are the volunteers prepared for their class sufficiently?Is there an organized resource room available for children’s ministry volunteers?Are there hands on supplies available in the classroom such as crayons, glue, scissors,paper, games, and DVDs?Can your children’s department comfortably accommodate special needs children?Are the volunteers reliable and ready to serve the kids when they arrive?
Administrative
Is there a follow up ministry for first time visitors?Is there policy and procedures written for the children’s department?Is there an application screening process for all volunteers (application, references,background check)?Is there a volunteer handbook?Does your children’s department have a vision, mission statement and values?Is there a children’s department budget?Do you inform parents by mail or email regularly?Do you recognized birthdays for volunteers and kids?Do you record attendance for each child?Do you update your child information database weekly?Do you meet with your volunteers quarterly to cast vision, train and show appreciation?Is the children’s department office organized and clean for efficient management?
