How to Participate:
Consult your licensed heating contractor or plumber for a listing of models that areeligible for the High Efficiency Water Heater Rebate Program.Once your new natural gas, high-efficiency water heating system hasbeen installed, review and completethis rebate application. Then attacha copy of all dated receipts/work ordersthat document the purchase andinstallation. Make sure that these receiptsindicate the name, license #, address,and phone number of the contractor who completed the installation.Mail the signed rebate form withattached receipts to:

Program Guidelines:
•
You must be a residential customer ofYankee Gas Services Company,Connecticut Natural Gas, or SouthernConnecticut Gas.
•
All installations of high-efficiencywater heating systems (indirect or on-demand tankless) must be installed by alicensed contractor and/or plumber. Allinstallations must conform to all applicablecodes and be installed in accordancewith all rebate Program Guidelines.
•
All proof of purchase receipts, work ordersor other documentation must include thecontractor name/license#/address/phonenumber, installation cost, and an itemizeddescription of the equipment, includingmanufacturer, model number, and AFUE(Annual Fuel Utilization Efficiency) or EF(Energy Factor) rating, as applicable.

All installations are subject to verificationthat the equipment has been installedand is operational.

Please read all Terms and Conditions onthe reverse of the rebate application.
