This is to inform you that your offer for the subject work is hereby accepted for a total value of Rs.24,95,988/-. (Rupees Twenty four lakhs ninety five thousand nine hundred and eighty eight only) in accordance to the special conditions of contract and schedule of rates and various conditions stipulated by the Railway in the tender document and signed by you. Payment will be made monthly basis (i.e Rs.207999/- Per month).

SCOPE OF WORK eContinuous dry sweeping and wet moping of floors. wet cleaning of all the wards, and toilets and also collection and removal of garbage to garbage collecting area. ~ Mechanized cleaning in all suitable areas '" Washing of stains, spit marks, betel nut stains and cleaning of water coolers, etc. OJ Cleaning of patient care articles like bed pan, urinal, spittoons, etc. Assist in collecting and sending urine I motion / sputum samples and taking up to laboratories. Assisting ward matron I nursing staff, in all patient care activities. '" " Cleaning & dusting of all furniture. Cleaning of glass panes, ventilators, lofts, r-;:jnshades, drains, etc . AREA OF WORK

~'*** Since the New OPD Block is not yet commissioned, only ninety percentage of the monthly payment should be claimed i.e 187199/- {2.07,999i- - 20,800/- (10%)} . As soon as the New OPD Block commences activities. the tOO% of be payment will be claimed.Iie. Rs.2,07,999!-)

The distribution of staff in shift basis must be as follows.
