For approved college credit courses not taken as a requirement of CH, the maximum calendar year benefit is two thousand dollars ($2000.00) for a full-time employee and one thousand dollars ($1000.00) for a part-time employee as outlined in the procedure.
PROCEDURE FOR REQUESTING REIMBURSEMENT:
Full-time and part-time employees are eligible to apply for educational reimbursement upon employment. An employee must complete 90 days of continuous employment before being eligible for educational reimbursement. 1. Applications are available from Human Resources or the CH intranet. The tuition cost per credit hour must be included on the application along with tuition cost documentation from the school. 2. Applications must be approved by the Director or Vice President and received/approved by Human Resources prior to the beginning of the course(s). Any applications received after the course begins will not be eligible for reimbursement. 3. Application coursework that may be approved includes accredited college credit courses directly related to CH opportunities. Programs not related to CH opportunities are subject to the approval by a representative of Human Resources and a Director, Manager or Supervisor from a department that would benefit from such a course. 4. If approved, 100% of the actual tuition cost will be paid, up to a maximum of $2000 in any calendar year for full-time employees and up to a maximum of $1000 in any calendar year for part-time employees. reimbursed. 5. Proof of successful completion of the course(s) will be required. 6. Within 30 days of completion of the course, the employee must submit to Human Resources an original grade report, noting successful completion of the course (C or above for undergraduate coursework, B or above for graduate or above coursework) and the receipt for tuition. Human Resources processes the reimbursement and issues a check to the employee for the amount eligible. 7. Carondelet Health requires a commitment of one year of employment after the course (s) is completed. If the employee leaves employment of Carondelet Health within one year following completion of the coursework being reimbursed through the educational reimbursement program, the employee is responsible for repayment of such monies. The amount to be repaid is prorated in twelfths according to the number of months employed after completion of the course. 8. Courses other than those that require traditional classroom attendance will be reviewed on an individual basis.
