1. Inform your YLP that you are going to apply for UPCAT through your batch site. Your name will be immediately forwarded to Ms. Trin and the Records Office in completion of UPCAT Form 2 (Secondary School Record). 2. Download and print (back to back) ONLY FORM 1 (UPCAT APPLICATION FOR FRESHMAN ADMISSION). Please be reminded of the following: 2.1 Use only black pen in completing the form. 2.2 Write in print and legibly. Avoid erasures as possible. 2.3 Leave the item unanswered if you’re not sure of what to put. 2.4 For item no. 8 (course code), refer to the General Information Bulletin (http://upcat.up.edu.ph/index.html) 2.5 For item no. 15 (sources of funds), ask assistance from your parents/guardians in completing this part. 3. Prepare your four recent 2x2 picture in white background. It should be decent (no sleeveless) and identical. Cropped and printed pictures on paper are not allowed. Make sure to write your complete name at the back of the photo. 4. Pay the UPCAT Application Fee of 450php. (reference: Sec. III. Modes of Payment). The application fee may be paid through any of the following (payable to UPCAT): Any Land Bank (LBP) branch, Account No. 1462-2220-13 (only for peso payments); or Any Philippine National Bank (PNB) branch, Account No.393496000021 (for peso payments) or Account No. 393496000039 (for US dollar payments); or Manager’s/Cashier’s Check/s (MC/CC) preferably from any of the following: •Allied Banking Corporation •Banco de Oro Universal Bank •Bank of the Philippine Islands •China Banking Corporation •Development Bank of the Philippines •Land Bank of the Philippines •Metropolitan Banking Corporation •Philippine National Bank •Prudential Bank •Rizal Commercial Banking Corporation •Security Bank Corporation •Union Bank of the Philippines •United Coconut Planters Bank NOTE: (1) Landbank and PNB will be charging a service fee of P25 for every application; (2) different banks have also set their own fees for the purchase of Manager’s Checks and Cashier’s Checks. 5. The following has to be submitted to your GUIDANCE COUNSELOR on JUNE 13, 2013 (Thursday), FIRST day of school:  Accomplished UPCAT Form 1 & 2  4 pcs. ID pictures 2x2  Copy of the bank payment slip 6.
