Breast malignancies are the most common malignancy in females • Infilterating duct carcinoma is most common histologic pattern seen. • It arises from ductal epithelium. • It mostly presents as a palpable mass but by the time the tumor becomes palpable about 50% have axillary lymph node metastasis. • 25% of familial cancers attribute to two highly penetrant autosomal dominant genes BRCA1 & BRCA2.
– The tumor is irregular ,hard ,varies in size that cuts with grating sound. – The cut surface is greyish to yellow in color with chalky streaks of stroma and small foci of calcification . – A part of tumor always extends irregularly into surrounding fat.
• No regular or uniform pattern seen throughout the lesion. • The malignant cells may arrange in cords acini sheets tubules of glandular epithelium. • These mixture of elements of various shapes are invading into diffuse fibrous stroma.
• Cytological details of tumor varies from small cells with moderately hyperchromatic nuclei to huge cells with large irregular hyperchromatic nuclei . • Frequently invasion of lymphatics and perineural spaces may be seen.
