Issue of Senior Citizens’ Identity Cards 1. Application Form (Form SW-15) 2. Application Form duly filled up alongwith following self attested documents:• Two passport size photographs. • Residence Proof for the last three years duly attested (Voter Identity Card, Ration Card, Electricity Bill) • Age Proof (Birth Certificate/ School Certificate/PPO) 3. Applications are received in the office and also in e-Sampark Centres. 4. In the e-Sampark Centres, Senior Citizens Identity Cards are issued on the spot. 5. Applications are considered on first-cum-first serve basis. Eligibility The Social Welfare Department is issuing Identity Cards to the Senior Citizens who are residents of Chandigarh are above 60 years of age. Proof of residence, age and two attested passport size photographs are required to be submitted with the application form. Following facilities are available to the holders of these Identity Cards:i. ii. iii. iv. v. 50% concession for travelling in CTU buses in Chandigarh. Separate queues for Senior Citizens’ in all the OPDs in General Hospital, Dispensaries, Poly Clinic and PHCs. Separate queues for Senior Citizens in Estate Office and other Public Places. Library facility in Community Centre, Sector 35, every day for two hours from 4.00 PM to 6.00 PM. Any other benefits extended to Senior Citizens by the Chandigarh Administration from time to time.

Now, Senior Citizens’ Identity Cards are also being issued through e-Sampark Centres set up by the Chandigarh Administration.
