Multiaxial Assessment

Axis I Clinical Disorders Other Conditions That May Be a Focus of Clinical Attention For reporting all the various disorders or conditions in the classification except for the personality disorders and mental retardation (which are reported on Axis II).

Axis II

Personality Disorders Mental Retardation Can also be used for noting prominent maladaptive Personality features and defence mechanisms.

Axis III

General Medical Conditions Current general medical conditions that are potentially relevant to the understanding of management of the individuals mental disorder.

Axis IV

Psychosocial and Environmental Problems Psychosocial and developmental problems that may affect the diagnosis, treatment and prognosis of mental disorders (Axes I and II).

Axis V

Global Assessment of Functioning (GAF) Overall level of functioning using the GAF scale.

Axes I and II make up the mental disorders per se; Axis III lists any physical disorders believed to bear on the mental disorder in question Axis IV lists any psychosocial and/or environmental problems that the person has been experiencing Axis V rates the persons current level of adaptive functioning (GAF)
