Enteric pathogens part one

Dr A.Aziz Djamal MSc.DTM&H.SpMK(K)

Enteric Bacteria All bacteria that normally found inside GIT

Many bacteria families, eg : Enterobacteriaceae. Vibrionaceae. Pseudomonaceae. Helicobacter/ Spiral like bacteria

Escherichia coli
Theodor Escherich 1883 Bacteria coli commune Family : Enterobacteriaceae Genus : Escherichia Spesies : Escherichia coli O antigen , H antigen, K antigen Originally Normal inhabitant of GIT 1980- ahead some pathogens

Escherichia coli pathogen

E.Coli : Normal inhabitant bacteria in GIT ( Intestinal & Colon rectum ). Outside GIT --- cause infection : UTI Other Tissue infection.

E.Coli pathogen
EPEC ETEC : Lt St EIEC EHEC EAEC

EPEC : Enteropathogenic Escherichia coli

Neonatal infection. Shiga like toxin. Adhesin/ lack of pili. Bloody diarrhea Attchment and Effacement-inflamatory Actin derangement

ETEC : Enterotoxigenic Escherichia coli.

Produce enterotoxin / intestine exotoxin A.Lt ( labile toxin ): Big molecule Antigenic. Activate Adenylate cyclase enzyne. Cholera like Severe secretoric diarrhea.

B. St ( Stable toxin ). Smaller molecule. Non-Antigenic. Mild diarrhea. Activate guadenyl cyclase enzymatic.

EIEC ( Enteroinvasive Escherichia coli ) Shiga like toxin produce. Necrotic on mucosal surface Bloody diarrhea/Dysentery baciller.

EHEC ( Entero haemorrhagioc Escherichia Coli) Haemolysin producing. Verotoxin producing. Cattle and Dairy product related. Uremic Haemolytic Syndrome. Food borne infection. O157H7

EAEC ( Entero adherent Escherichia coli ) Aggregation E. Coli on GI tract Mucose. Less watery Diarrhea. Flagella mediated. Malabsorbtion syndrome..

Diseases : Gastreoenteritis UTI. Meningitis. Uremic Hemolytic Syndrome.

Problem : ESBL ( Extended Spectrum Beta Lactamase ) Resistant to Cefotaxime, Ceftazidime, Ceftriaxone. Plasmid mediated.

Vibrionaceae
Curve rod/ comma shape One polar flagel Gram negative Marine bacteria-Halophilic bacteria Marine biota aas reservoir. Genus : Vibrio Spesies : V. Cholerae, V. Parahaemolyticus, V.mimicus, V. damsela

Vibrio cholerae
Water borne disease. Strong enterotoxin ( choleragen ). Ctx-gene. Secretory watery diarrhea-dehydration-electrolyte Adenylate cyclase stimulation. Classical ( O1) Pandemic. O139 in Asia. Biotype : Vibrio El Tor

Vibrio parahaemolyticus
Food borne( Sea food). Kanagawa phenomen/Hemolysin (tdh and trh) Diarrhea ( Bloody ). Abdominal cramp. Shelfish

Klebsiella
Fam. Enterobacteriaceae Spesies K. Pneumoniae K. Oxytoca. K.rhinosclerotica

Capsulated bacteria. Capsule : Polysaccharide polymer. Antiphagocytic activity. ESBL plasmid carrier. Respiratory Infection ( pneumonoiae )

Oher enteric pathogens Proteus Enterobacter Yersinia All Enterobacteriaceae family
