Please briefly describe the extent of your proficiency and experience in the procedures highlighted

above, along with any particular clinical skills/experience/special interests you possess that you may
wish to highlight.

As a doctor in FY1 post and especially when I am on call, I have had the opportunity to do most of
these procedures.
I am a quick learner therefore with minimal supervision I was able to master the techniques of these
procedures and to date I have independently performed a number of the above procedures and I am
confident that am able to do them without any issues.

In the context of this post, in reflecting on your own skills and abilities, are there any areas where
you might seek further development and support?
I have performed only a small number of urinary catheterizations and I usually require the
assistance of another, especially in places where I would need help to assure an agitated patient
when am trying to perform the procedure. Also to adhere to aseptic techniques, I require another
person to hand me materials to avoid contamination.
Therefore I would like to perform more of these to improve on my technique and also practise doing
it without the help of another person

Please provide details about your teaching experience
During my final year, I was assigned first year students to teach and help them with their studies,
which was an enormous success as all of them overcame the tough challenges encountered in the
first year. I have found that explaining relatively complex topics in a simple, logical way demands
empathetic skills not generally required in seminar discussion. Also, I was asked several times by my
peers to explain complicated topics in an easily understandable way. I have made several successful
PowerPoint presentations, for example in the classroom as well as to my mentor at The Mayo Clinic.
In addition, I was employed as a part-time chemistry teacher at a private tutorial centre in Kingston,
London, between 2005 and 2006.

Experience learnt from audit
I have learnt that the primary responsibility for ensuring that an audit/ research in serving its
intended purpose rests with the person who is doing the audit, it depends on how well an auditor
plans and conducts the project, by effective utilizing of time to prepare for an audit, establishing
and maintaining a professional atmosphere, identifying common auditing/research pitfalls,
communicating results and appropriate follow up actions.
I have learnt that a definition of auditor means someone who examines with the intent to verify.

Management and Leadership Experience
Management and Leadership Experience
Please highlight experience you may have which is relevant to this post, not necessarily
limited to professional activities
As a team leader in retail, I highly valued good teamwork and gained confidence in negotiating with
interested parties to solve logistical and personal problems.
I have also been the person of choice when it comes to leading a project done by the class. I had
successfully allocated fellow colleagues to undertake certain tasks and finally analysed them and put
together a power point presentation in my 5th year for Social medicine and Hygiene.
I have also been in charge of selling tickets and decorations committee for our annual international
students gathering in the year 2011. We had raised approximately 15 000 czk which covered all the
expenses for the event.
I have taken an active part in organizing class get together every month, which also included
organizing games, this way it provided an opportunity for us to get to our fellow peer students.

Describe situations where you have been involved in working in a team, not necessarily limited to
professional activities
Throughout my life I have appreciated the results of, and been involved in, a wide range of
teamwork situations. Recently, these have included cooperation at work, during research, on ward
rounds and in the organisation of events such as graduation nights, and other ceremonies. I enjoy
collaboration. For me, the benefits achieved through mutual support, respect, and different minds
being focused on the same problem include increased efficiency and a greater output than is
possible from individuals alone, in addition to a feeling of shared responsibility and joint
achievement. As an example of this, while working in the care home I experienced that teamwork

My voluntary work at Kingston Hospital necessitated assisting with patients needs and giving
administrative support to the nursing team. These activities enhanced my ability to use appropriate
organisational and time management skills to anticipate and meet the demands of these groups.
The activities involved in being a Health Care Assistant at St Clouds Nursing Home during medical
school holidays taught me not only about the care, vulnerability and demands of elderly patients, but
also about some of the challenges faced at the very basic level of health care, and this experience
gave me confidence in working with the elderly and increased my respect for time management skills.
Working in retail to financially support my studies helped me hone other valuable interpersonal and
organisational skills. As I was a member of management, I faced many challenges and came up with
new ideas to achieve targets.I have up to date competence with information literacy and information
technology.

I am a hard-working individual and have learned to pace myself to work a long day and still be fresh
for study and research in the evening. Keeping fit has contributed to this. I play cricket when I have
the opportunity

I gained a scholarship to take part in research in a prestigious institution, The Mayo Clinic,
Rochester, Minnesota USA. I had a relatively short period of time but I aimed to publish my
research. I worked so hard almost day and night to achieve this success.

Being generally and empathetic and understanding person, I feel i can devote my time to
completely understand a patient's problem and think intellectually to mange their issues in an
appropriate manner.
I am known to be a patient person and i feel this is a very important quality in a physician,
sometimes patients do not always tell precisely about their symptoms, therefore it is
necessary to let them explain their symptoms in their own words and it is our duty to grasp the
relevant information and use it in patient management.
I am punctual to work, i prefer being at work few minutes early than late.During my FY1
training, I make it a point to come to work early. I review all the patients on my list, any events
during the night, any changes in management,I discuss with the nursing staff who took care of
these patients over the night and i inform this to my senior doctors and consultant. I beleive
that staying on top of our work , good communication and docuementation skills leads to less
errors and better patient management.
