This was an orientation week. You did arrive for clinical on time and prepared for the day. You demonstrated CBG
today with adequate technique..RSF

Second week of orienting to the clinical process. You made an unoccupied bed and took a set of vitals. It did take
over 15 minutes for you to take a set of vitals so I would like you to continue to practice this skill frequently in order
to become more comfortable and efficient at the processRSF

First care plan submitted today. Your care prep was complete, please see the corrected version you will receive at
week 4 clinical. Your evaluation of your nursing diagnosis and interventions was very well thought out, it is obvious
you reflected on this experience. This week you were checked off on assessment of head and neck and cardiac.
Remember to avoid using a sing-song voice when speaking with patients and though you need to inform the
patient of what you are going to do, you do not need to provide a lengthy description. In other words, you can say,
please squeeze my hands rather than, now I am going to test your strength by asking you to squeeze my hands.
You did this repeatedly through your assessments; your patient will grow tired with such lengthy experiences with
you. Practically, you will not have this kind of time with a patient, so remember, kind, respectful and efficient.RSF

Continue to work on the flow of your head to toe assessment and your skills. Remember to avoid talking too much;
especially announcing each task you are going to do. Work on condensing your assessments, there are several areas
you double charted (you talked about skin under skin, then under respiratory, cardiac, head & neck, etc.) You only
need to talk about skin once in the skin assessment. Chart edema either under skin or cardiac, not both, etc. Also
work on condensing your DAR note. You want to be efficient and accurate, not wordy. Overall you are demonstrating
growthRSF

Overall a nice job on assessments, please see my comments for feedback. I noted less double charting this week so
very good notation of my feedback. Your DAR note was well written. You did not evaluate your second nursing
diagnosis/interventions. You did not have any check offs this week. Well written patho that included an integrated
