An inspector may be assigned fulltime tothe job by the County Engineer at the expense of the Permittee to protect the interests of the County 1. Excavation made in the pavement area of the roadway shall be backflled with controled density fil then topped with asphalt base and surface course, per Summit County Engineer pavement replacement standard, Excavations made within eight (8) feet ‘of the pavement edge, shall be backfilled witn 304 imestone matarial in sc (6) inch layers, loose measurement. Each layor shall be tamped or rolled. In all cases, back‘iled material extending more than six (8) inches above the level ofthe sides ofthe trench, ‘must be removed from the job site. Any pavernent cul, damaged or undermined by excavation, shall be removed and replaced to the satisfaction of the County Engineer, Temporary pavement replacement shall be maintained by the permittee in good ‘condition. Permanent replacement must be completed as soon as practicable 2. Excavation made within the limits ofthe highway (other than described above) shal be restored equal to or better than existed prior to excavation @s soon as possible. 3. Permittee shall notiy the office of County Engineer (390-643-2850), atleast twenty-four (24) hours prior to starting work 4. copy ofthis permit and plan of work must be on the job and available to the County Inspectors or work may be stopped. 5. The permite agrees to provide barricades, lights, waming signs, unformed officers, or lagmeen, as necessary, to protect the ‘traveling public and agrees to save the County free form all damages and labilties arsing from the work done under this permit ‘and to comply with county construction zone policy. 6. No‘offhe-road” equipment with ste! tres, treads or cleats will be permitted on pavement without proper planking, 7. The top of any pipe line crossing the highway must be a minimum of ffteen (16) inches below grade of existing gutter and four (4) fee, six (6) inches below pavement crown grade, 8. The contractor shall comply withthe current standards and specifications of the County Engineer. 9, The contractor shall notty the Utities Protection Services at 1-800-362-2764 atleast forty-eight (48) hours prior to the start of ‘construction, 10. The contractor shall comply with ll State and Federal Safety Regulations for the construction site and agrees to save the County free from all damages and liabilities arising from non-compliance with the safety regulations. 11. ‘The contractor shal! notify the local police, fre department, schools and any other affected agencies forty-eight (48) hours prior to the start of construction 12, The fee as indicated above is non-refundable when once the Road Opening Permits granted
