Background

1. This pertains to the Application Of Davao Memorial Park, Inc.


Represented By Mr. Enrico Reyes Valencia, For Alteration Of Plan (Alt)
For The Establishment Of Its Columbarium Located In Matina, Davao
City;
2. Davao Memorial Park, Inc., a private corporation, duly incorporated
under the laws of the laws of the Philippines having its domestic and
its principal place of business here in Davao City;
3. The area applied for alteration is comprising one thousand six hundred thirty
(1,630) square meters;

4. The purpose of the alteration is to_________ (I am not sure. It was not


indicated in the documents);

Highlights
5. The Office of the Housing and Urban Development Coordinating Council
Housing and Land Use Regulatory Board, in its evaluation stated that Davao
Memorial Parks expansion is in conformity with the Implementing
Standards, Rules and Regulations for memorial park pursuant to Executive
Order No. 646 dated February 07, 1982. Thereafter, it issued a Development
Permit in favor of Davao Memorial Park;
6. Affidavits of No Objection were also executed by different lot owners in
Davao Memorial Park and attested their properties are adjacent to the
proposed Columbarium Project of the Davao Memorial Park, Inc;
7. That the former executed their affidavits to inform the Honorable
Sangguniang Panlungsod and all concerned agencies that they interpose no
objection to said project neither its establishment on said property and that
they are even making known to the public, through their sworn statements,
their conformity to the same;
8. Majority of the homeowners conformed to the alteration of plan as shown by
their above-mentioned affidavits of objection;

9. At the Committee Hearing of the Sub-Committee on Housing and Subdivision


Development under BP 200 (Medium-end Projects) last July 24, 2014, no
objection was raised with regard to this application;
Considerations
10.The proponent submitted the following documentary requirements relative
to their application, for the perusal of the Sub-Committee of Housing and
Subdivision Development Under BP 220 (Medium-end Projects):
a) City Housing and Land Use Regulatory Unit Findings and
Recommendations Sheet;
b) City Housing and Land Use Regulatory Unit Inspection Report;
c) Written Conformities to the application for alteration of plan by lot
owners adjacent to the proposed Columbarium of the Davao Memorial
Park;
d) Copies of the plan showing the proposed alteration duly signed and
sealed by a licensed Architect/ Engineer;
e) Letter
stating
the
proposed
reason
for
the
proposed
alteration/conversion
f) Sworn statement that the affected lots for the alteration have not been
sold;
g) Certified true copy of the title(s) of the affected lots/units of the said
lots/units;
h) Other pertinent documents relative to the request;

11.The Committee recommends to Grant the application of Application Of

Davao Memorial Park, Inc. Represented By Mr. Enrico Reyes Valencia,

A RESOLUTION GRANTING THE APPLICATION OF OF DAVAO
MEMORIAL PARK, INC. REPRESENTED BY MR. ENRICO REYES
VALENCIA, FOR ALTERATION OF PLAN (ALT) FOR THE
ESTABLISHMENT OF ITS COLUMBARIUM LOCATED IN MATINA,

WHEREAS, this pertains to the Application Of Davao Memorial Park, Inc.

NOW THEREFOR, be it resolved as it is hereby resolved that a resolution be
passed to grant the application of 11. The Committee recommends to Grant the
application of Application Of Davao Memorial Park, Inc. Represented By Mr. Enrico
Reyes Valencia, For Alteration Of Plan (Alt) For The Establishment Of Its
Columbarium Located In Matina, Davao City;
RESOLVED FURTHER, that copies of this Resolution be furnished to the Office of
the City Mayor, the City Legal Office, the City Planning and Development Office and
all other offices/departments concerned, for their information, guidance and
appropriate action.
