At the request of Dr. Rebecea Hamilton, District 21 Medical Examiner, I began an
inventory of the evidence room of the fecilty. During this inventory I eame across the
partial remains of Alex Sosa (784-06). These remains consisted of bone and charred bone
fragments of feet, the maxilla and mandible, an individual tooth, and six containers of
‘bone fragments. These remains were beside other osteological remains donated to Dr.
‘Heather Walsh-Haney of Florida Gulf Coast University. I reported the discovery to both
Dr. Hamilton and Dr. Robert Pfalzgraf, MD. Dr. Palagraf, Deputy Chief, Medical
Examiner asked what was the final disposition of the other remains? Inspection of the
‘computer record had the remains released to Fort Myers Memorial Gardens. No
‘cremation authorization was on file. He stated we would have to offer the remins back
tothe family. He stated he would contact Dr. Walsh-Haney to ascertain a reason why the
remains may have been retained, The remains were again photographed and place into
‘the intake cooler

Inspection of the computer record listed the disposition of Alex Sosa remains as(4).
‘Being unfamiliar with that computer designation I checked it’s meaning, a (4) ithe
designation for donation, [inquired with the administrative staff why those remains
‘would be listed as donation and they stated it must have been @ mistake. They stated the
family was quite adamant about getting the remains back for burial. I checked a copy of
the death certificate which also listed the final disposition as burial. I then changed the
‘computer designation back to 1) for burial
