Practice Guidelines
I want to practice and develop performing full
assessments on residents while including pain as an
essential vital.
GOAL #1: ACTIVITIES AND TIME FRAME
1) By March 6th have preceptor evaluate assessment
skills performed on a resident
2) By March 8th will have researched appropriate
assessment steps and guidelines
3) By March 11th will have practiced all assessments
fully on residents
4) By March 14th will have had preceptor evaluate all
assessments and give feedback
5) By March 16th will have implemented feedback in
nursing practice
LEARNING GOAL #2

Documentation, Revised 2008 Practice Guidelines


I want to enhance current documentation skills to
ensure all information is present and in an easy to
follow manor.
GOAL #2: ACTIVITIES AND TIME FRAME
1) By March 5th have preceptor review previous
documentation and add input
2) By March 7th have preceptor proofread all
documentation before saving it to ensure all
information is easy to follow and present
3) By March 10th research examples of good
documentation techniques with the CNO and RPNAO
4) By March 11th implement researched techniques in
documentation and have preceptor oversee
5) By march 15th enroll in RPNAO online
documentation course for April 10th 12:00-1:00 pm

Therapeutic Nurse-Client Relationship Practice
Guidelines
I want to develop an in service on my unit to help
both staff and myself achieve clear therapeutic
communication with the residents
to reduce potential errors and enhance trusting
relationships with residents on the unit
GOAL #3: ACTIVITIES AND TIME FRAME

1) By March 8th will have reviewed "Therapeutic


Nurse-Client Relationship Practice Guidelines"
2) By March 10th will have reviewed Leisure WorldsBrampton Woods policies and procedures in regards
to safe communication
3) By March 12th will have practiced therapeutic
communication with residents as well as family
members
4) By march 13th will debrief with preceptor in
regards to newly obtained communication methods
and their efficiency
5) By March 17th will have a mini staf in-service on
floor post report on therapeutic communication
methods and techniques Collaboration with team is
key to build together.
HOW DO MY LEARNING GOALS SUPPORT MY
COMMITMENT TO CONTINUING COMPETENCY?
I am remaining competent by learning to perform a
full assessment on residents as well, how to
incorporate pain as a key vital sign when assessing
the resident. I am also learning about how to develop
strategies on therapeutic communication within my
practice to enhance patient care to the fullest. In
addition how to document clearly so all staf
understand client' status and health related changes
that occur. This newly obtained knowledge will aid in
the quality of nursing care provided as well as
promote the residents/families trust in myself, and
staf on the unit.
EVALUATION OF CHANGES/OUTCOME TO MY

PRACTICE
I believe that I have enhanced my day to day
practice by implementing newly obtained skills as
well as listening to feed back and making appropriate
changes as necessary. Because of these changes I
am able to complete full assessments on residents
without missing any key information or questions. I
am able to incorporate pain and PQRSTU into my
assessments in order to gain a full understanding of
the residents current situation. My documentation
skills has improved which results in these
assessment and other pertinent information being
easily transmitted to other team members. I believe
that my knowledge, skill and judgment has enhanced
tremendously because of constant learning.
