HSSE MONTHLY REPORT

LEKKI OFFICE – DOMGAS PROJECT


MONTH: NOV, 2009
HEALTH:

1. No work related or occupational health illness was recorded within the month.

2. Health Hazards Observed: NIL

SAFETY:

PPE
10nos redwing’s safety shoes, 10nos Eye Goggles, 10nos fire retardant coveralls
monogrammed with company logo, 10nos hard hat fitted with ear muffs and chin straps
were procured for site visits within the month
Stock at end October:
Total number of PPE in stock at end Nov. 2009 include:
Coveralls (6), Eye Goggles (6), Safety shoes (6), hard hats (6).

ACTIVITIES
• Conduct of daily TBT for the drivers, cleaners and security operatives
• Conduct of induction training for new employees, clients//visitors.
• Daily update of the HSE performance board
• Carried out bi-weekly inspection of the offices/guest houses
• Follow-up/close out of outstanding client inspection findings
• Follow-up/close out Unsafe Act (UA)/Unsafe Condition (UC) reported.
• Conduct of monthly HSE meeting for the month of September.

HSE TRAINING:
• Defensive Driving Course (DDC) for drivers – 4nos. METS drivers was
programmed for the training at ILLR Certification Office SHELL Lagos, and they
have successfully obtained SHELL (SNEPCO) driving permit.

TOOLS, GEARS AND EQUIPMENT:

• Nothing To Report (NTR)

INCIDENT REPORTS

• No /first aid case was reported during the month.

SECURITY:

• No incident/first aid case was reported during the month.

ENVIRONMENT

• Generally, there has been marked improvement in house keeping and it is on–
going..
• Spill of diesel at the diesel storage area

PLANNED ACTIVITIES FOR NEXT MONTH:

• Conduct of monthly HSE meeting for the month of October.

AREAS OF CONCERN:

• Inadequate car park area.


• Water treatment plant can only be powered by the generating set, thus no flow of
water when there is power supply from PHCN.

SUGGESTION(S) FOR IMPROVEMENT:

• Provide overhead water storage tanks on both buildings.


• Provide secondary containment (bund wall) around the diesel tank.
