Student: Derek

Brief Historical and Referral History:
Derek was having trouble in school with directions and focus. He
failed and had to repeat the third grade. The repeat did not help
with his understanding. His mother and father reached out to have
him placed in special education classes to aid in his academics. His
mother reports difficulty with focus and hyperactivity. His mother
feels that he needs one on one to help with these issues. His mother
also states that Derek has a history of ear difficulties, including a
ruptured eardrum.
The main concern:

average across all domains.
