JUDICIAL AFFIDAVIT OF RAMON T. TY
We are offering the testimony of the witness to prove that when
the motorized boat M/B King Arthur was seafaring the weather was in
good condition since the public storm signal no.1 raised was lifted by
DOST PAG-ASA on December 6, 2014 at around 11 AM.
This judicial affidavit was taken by ATTY. NIL E. VOSA, with
postal address at Bagong Silang I, Barangay Washington, Surigao
City, and was executed at his office 219 P. Reyes Street, Surigao
City.
I, RAMON T. TY, of legal age, Filipino citizen, and a Weather
Specialist, with postal address at Pepang Village, Surigao City, after
having been duly sworn to in accordance with law and with
knowledge that I may face criminal liability for false testimony or
perjury, do hereby depose and say:
Q1: Please state your name and other personal circumstances.
A1: I am RAMON T. TY, 52 years of age, Filipino, Weather Specialist
and a Chief Meteorological Officer I with residence at Pepang
Village, Surigao City.
Q2: As a Weather Observer, where are you connected?
A2: I am currently connected with DOST PAG-ASA here in Surigao
City.
Q3: What is your position in DOST PAG- ASA?
A3: I am a Weather Specialist. Currently, I am the Chief
Meteorological Officer here in PAGASA Weather Station Surigao City.

Q4: How long been appointed as Weather Specialist?

A4: I have been a Weather Specialist for almost 26 years.
Q5: What are your duties and responsibilities of as Chief
Meteorological/ Weather Specialist?
A5: I am tasked to monitor day to day weather condition by observing
weather condition by observing weather parameters in a 3 hourly
observations, except when there is a tropical cyclone entering the
Philippine Area of Responsibility (PAR) an hourly observations like
coding and encoding the temperature, pressure, wind, visibility, cloud
amounts, types of clouds are conducted. Also I manage and
supervise the subordinates of their timely and accurate observations
of the weather elements.
Q6: Where were you on December 6, 2014?
A6: I was in my office at DOST PAG- ASA.
Q7: What was the Weather Bulletin of Surigao City on December 6,
2014?
A7: As of 5:00 A.M. of December 6, 2014, Public Storm Warning
Signal No. 1 has been raised over CARAGA Region including
Surigao del Norte, Province of Dinagat Islands, and Siargao Island.
Q8: How strong was the typhoon?
A8: It has a maximum sustained winds of 55 kph near the center
Q9: Was there any other weather bulletin issued by your office on
December 6, 2014?
A9: Yes, we issued weather bulletin no.2.
Q10: What was the second weather advisory about?
A10: Our Weather Bulletin No. 2 indicated that the typhoon was about
to make landfall over Sorsogon and moving Westward towards Burias
Island. Leading the PAGASA to announce in 11 a.m. bulletin on the
same day, December 6, 2014, that Public Storm Warning Signals
(PSWS) raised in Surigao del Norte, Dinagat Province and Siargao
Islands were lifted.
Q11: Did you verify the truth and correctness of the said Weather
Bulletin No. 2?
A1: Yes Sir.
Q12: If the said weather advisory will be shown to you, will you
Able to recognize it?
A12: Yes Sir.

Q13: Showing to you this document dated December 6, 2014, can

you please go over it and tell this honorable court what is the relation
of this document to that you were referring to.
A13: This is the weather bulletin no.2 issued
Q14: Please go over the said document, there is a signature above
the written name RAMON T. TY, whose signature is this?
A14: It is my signature Sir.

and veracity of the foregoing facts which are based on my personal
knowledge and belief.
