This form is to be completed by someone other than a Counselor or
College/Career Center Specialist (i.e. teachers, administrators,
clergymen, supervisor, etc.). Family members or personal friends may
not complete recommendations.
Please feel free to attach a letter of recommendation with this form. The
letter must include a rating of the student for each of the Lam Core Values
below.
