Minutes of the Pre-Construction Meeting for the project, Concreting of
Jimenez Gata San Isidro Provincial Road held on April 12, 2016 at
OPPDC Function Room, Capitol, Oroquieta City

The meeting started at 10:00 in the morning with a prayer. It was
facilitated by Engr. Neil Lemon Mediodia. Engr. Mediodia thanked and gave the
participants a warm welcome. Engr. Mediodia then brought to the table the
agenda mainly on the smooth implementation of the project, Concreting of
Jimenez Gata San Isidro Provincial Road. From then on, Engr. Mediodia gave
the time to Engr. Hope Earl Ruelan for the presentations prior to the
implementation of the said project.
Presentation on the Implementation Requirement and Procedure
Before Engr. Ruelan started the presentation, he reminded that the
province is aiming to be one of the top performers of the KALSADA program and
with that, strict implementation, observance with the plans and specifications
and progress monitoring and reporting must be duly complied under the
timelines set by the program. Engr. Mediodia also reminded that the KALSADA
project implementation weigh 40% in the scoring criteria for the 2017 KALSADA
funding. Furthermore, Engr. Myrna Balaoro informed the body that among 74
provinces in the country, 24 provinces were invited in the first batch of KALSADA
Orientation given that these provinces have already started implementing the
said program.
On the information and publicity, dimensions of the tarpaulin together with
the other details to be found in the billboard was presented. Engr. Ruelan then
reminded Engr. Bonita to submit the construction schedule with S-curve. Engr.
Bonita responded that he will send a copy within the week. Engr. Ruelan then

continued with the responsibility of the PLGU that a Site Engineer must be a full
time in supervising the implementation on site, on a day to day basis. The
contractor should mobilize within seven (7) days after the receipt of the NTP.
Also, he highlighted that there is a participatory monitoring together with the
DILG and a regular conduct of coordination meeting every month. Engr. Bonita
agreed and made a note on the matters discussed.
On the responsibility of the contractor, he reminded that construction
materials shall be in accordance with the approved plans and specifications,
Materials Engineer shall be present on site, appropriate equipment are on site,
quality control tests are conducted consistent with the frequency and number
specified in the Technical Specifications for each item of work, and health and
safety standards set by DOLE are duly implemented. Engr. Bonita responded
positively.
Engr. Balaoro reminded Engr. Bonita that Health and Safety must be
strictly observe since the Commission on Audit gave Audit Findings in this regard.
Also, she asked Engr. Mediodia to accompany Mr. Samson Napigkit, one of her
staff in PEO, for the video- tagging on site.
Engr. Ruelan continued discussing with the progress monitoring and
reporting. It was agreed to have a coordination meeting every 25 th day of every
month. Also, accomplishment report is agreed to present during the meeting for
further validation. Final scanned copy of the signed report is then sent thru email
address of the DILG KALSADA. Furthermore, salient technical aspects of the
contracts were read during the meeting as a reminder to the contractor and the
PEO personnel. Also, he reminded that strict compliance of the contractor to
pertinent policies and guidelines must and at all times be observed. Engr. Bonita
asked for electronic copies of the templates/forms for them to use and Engr.
Mediodia then assured Engr. Bonita to have those sent in the email address he
provided.
Open Forum
Engr. Balaoro asked to have a Groundbreaking Ceremony since the NTP
was already issued. PPDO Teresa Calamba then responded not to have the
ceremony since it was not tackled during the Orientation Seminar in Subic,
Zambales.
Engr. Efren Baliog aslo reminded Engr. Bonita to conduct as soon as
possible the as stake survey since they have already started mobilizing their
equipment. Engr. Higinio Sarigumba shared the same and he urged that the as
stake must be joint with the PEO personnel. Engr. Bonita replied that he has to
consult the availability of their survey team and he will give a feedback within
the week.
The meeting ended at 11:32 in the morning.
