URINARY TRACT INFECTION

ASSESSMENT S: “Nurse, ang sakit sakit ng tyan ko” as verbalized by the mother. O: • • Facial grimace Guarding behavior

NURSING DIAGNOSIS Pain related to infection as evidenced by facial grimace and guarding behavior.

PLANNING After the nursing intervention the patient body temperature will verbalize relief of pain.

INTERVENTION INDEPENDENT: • Apply heating pad to lower back for back pain • Instruct the patient in use of sitz bath for perineal pain • Teach the patient divertional activities to reduce pain • Eliminate additional stressor and sources of discomfort whenever possible DEPENDENT: • Administer analgesic and antispasmodic s as prescribed.

EVALUATION The patient verbalizes relief of pain.
