ENROLMENT PROCEDURE

For FRESHMEN
EFFECTIVE SUMMER 2009

FLOW DIAGRAM DETAILS

1. Go to the Office of Admissions and Scholarships (OAS):


(OAS):
OAS
a. Fill - out the Enrolment Application Form
b. Submit the following requirements:
• Original copy of HIGH SCHOOL CARD
• Original copy of Certificate of Good Moral Character
• Original copy of Birth Certificate preferably NSO certified
• One (1) pc. – long size brown envelope
Accounting 2. Proceed to the Accounting Department and pay P150 for the testing fee and P50 for the
Department digital picture.
3. Proceed to the Office of Admissions and Scholarships (OAS):
(OAS):
OAS a. Present the receipt of payment for picture taking and schedule of Entrance Exam.
b. Take the Entrance Exam at the Testing Room/ Guidance Office upon presentation of
the Testing Permit (detached portion of the Enrolment Application Form).
c. Give the countersigned Testing Permit to OAS and wait for the test result, interview
and issuance of the duly signed enrolment admission slip (pink slip).
d. For Nursing applicant, go to the College of Nursing Office for physical examination.
NOTE: The OAS should ensure that the personal information of the freshman applicant
has been encoded before releasing the duly signed pink slip and the test result.
4. Go to the Enrolment Technical Office (ETO):
(ETO):
ETO a. Present the pink slip for the issuance of your Student ID Number
b.1. For BLOCK SECTIONS (Those who will take regular load of first year, first semester):
• Approach the enrolment adviser for the encoding of your tentative-subject load. This
will serve as your reservation for 24 hours.
• Wait for your computer-generated enrolment form duly signed by the adviser.
b.2. For NON- BLOCK SECTIONS (Those who will not take regular load of first year, first
semester):
• Get your schedule of classes from the postings in the bulletin board.
• Fill- out the tentative subject load form with your tentative subject load and present it
to the enrolment adviser for encoding. This will serve as your reservation for 24 hours.
• Wait for your computer-generated enrolment form duly signed by the adviser.
5. Proceed to the Accounting Department for payment.
Accounting
 Students who will pay in full are entitled to a cash discount which will be evaluated by

 Brothers and sisters are entitled to a tuition fee discount.

ETO 6. Proceed to the Enrolment Technical Office (ETO):


a. Submit the remaining copies of your computer-generated enrolment form.
b. Claim your Temporary Study Load. Check immediately if your study load contains all the
subjects you intend to enroll and the correct schedule of classes. Changing and
dropping of subjects are discouraged. However, changing of schedule due to erroneous
encoding is allowed only within 3 working days from date of issuance of temporary
study load.
7. Go to your respective Records In-Charge at the Registrar’s Office on or after the date

issuance of the duly signed computer-generated enrolment form.

Accounting 2. Proceed to the Accounting Department for payment.


Department  Students who will pay in full are entitled to a cash discount which will be evaluated
by the Accounting Department.
 Brothers and sisters are entitled to a tuition fee discount.

3. Proceed to the Enrolment Technical

4. Go to the Registrar’s Office on the date specified in the temporary study load and claim
Registrar’s your official study load upon presentation of ID card for validation. Failure to claim

b. Surrender your ID card.
3. Go to the College/ Department where the course you wish to enroll
belongs and see the DEAN/CHAIR for the following:

Department b. Preliminary evaluation of your subjects taken from former program/course using the
prospectus/ checklist of subjects as basis of your tentative subject- load.
c. Encoding of your tentative subject- load which will serve as your reservation for 24
hours.
d. Printing of your computer- generated enrolment form duly signed by the Adviser upon
presentation of final admission slip.
e. Attachment of the prospectus used by the dean/ chair for preliminary evaluation of
subjects taken from other programs to your enrolment form.
Accounting 4. Proceed to the Accounting Department for PAYMENT of the following:
Department a. Tuition fee (separate receipt)
b. ID card (separate receipt)
NOTE:
 Students who will pay in full are entitled to a cash discount which will be
evaluated by the Accounting Department.
 Brothers and sisters are entitled to a tuition fee discount.

b. Sign an UNDERTAKING for OFFICIAL ACCREDITATION of subjects taken
from your previous program/ course (see Important Reminders #3 below) using
Registrar form 23.
NOTE: General Education subjects with the same descriptive title and units do not
need to be accredited.

• Informative Copy of TRANSCRIPT OF RECORDS
• Honorable Dismissal/ Transfer Credentials
• Original copy of Certificate of Good Moral Character
• Original copy of Birth Certificate preferably NSO certified
• One (1) pc. - long size brown envelope

b. Give the countersigned Testing Permit to the SAO Head for interview and approval of
your enrolment admission.
c. Get your Student ID Number.
5. Go to the College/ Department where your course belongs:
College/ a. See the Dean/ Chair for the approval of your Enrolment Admission.
Department b. Have your subjects taken from other schools initially evaluated by the Dean/Chair using
the prospectus/ checklist of subjects as basis of your tentative subject load.
c. Present your tentative subject load to the enrolment adviser for encoding. This will serve as
your reservation for 24 hours.
d. Wait for your computer-generated enrolment form duly signed by the Adviser.
e. Attach the prospectus used by the dean/ chair for preliminary evaluation of subjects
taken from other schools to your enrolment form.

8. Go to the Accounting Department and pay P2,050.00 for the enrolment fee and
Accounting P300 for Testing Fee.
