Go to www.ssa.gov. You will see three buttons in the center of the screen. The first allows anyone who s ready to retire the ability to apply on-line; the second is to apply for a Disability Benefit(s) and the third is there so folks can order a Social Security Card.

Review the Adult Disability Checklist for details about what you will need before starting the online process. This checklist is like an inventory. Use it to check off information as it is gathered. Fill out the online Disability Benefit Application to provide SSA with information regarding eligibility for payment. Answer the questions accurately and completely. The more information you re able to give, the better equipped the disability analyst is to make a favorable decision. Fill out the online Adult Disability Report to provide us with medical and work history. SSA is looking for an inability to function normally on a day-to-day basis. The answers should indicate that the claimant cannot engage in Substantial Gainful Activity for at least a year and for an indefinite period of time after that. Provide as much detail as possible. Names, addresses, phone numbers, etc. accurate lists of medications and their dosages, any resulting side effects; when answering these questions keep in mind your worst day. Fill out, sign and mail or take the Authorization to Disclose Information to the Social Security Administration to your local Social Security Office. Whenever possible take the paperwork to the Social Security Office and get a copy of whatever you submit and a stamped receipt proving you were say and what services were provided to you. Then put that receipt in a safe place, filed by date, the most recent on top. That will help to establish an accurate benefits history.
