This form should be completed by two proposers: staff, students or alumni. Where the nomination is self-initiated, the form should be signed by the nominee and seconded by another proposer. Separate forms for the same nominee will be accepted. A valid nomination will have at least two proposers (or a self-nomination plus an additional proposer). For the purposes of this award scheme a team is made up of two to ten individuals who have worked together to achieve a common goal that supports student learning.

Nomi tions shoul include a short statement (no more than t o sides of A4) gi ing a critical assessment of the work for which the candidate is being nominated, against the relevant criteria (set out in item 3 of the President¶s Awards for Teaching Excellence Guidelines). Completed nominations, marked µPresident¶s Awards for Teaching Excellence¶ should be submitted to Bernadette Henchy (bernadette.henchy@nuigalway.ie) in the Centre for Excellence in Learning and Teaching ( CELT) before Friday, March 4th, 2011.
