MEET OUR YOUTH ENRICHMENT INSTRUCTORS

Cafe Amigos Amigos Program Cafe Amigos is a Spanish lunch bunch! Following lunch children will enjoy games, songs and playtime while learning Spanish! The instructor is a native speaker and trained to work in early childhood. Children bring own lunch and a beverage. Staff will escort children from the Winnetka Community Nursery School.

Extraordinary Kids curriculum and materials have been personally created and developed by directors Carol Modes and Susie Blau. Modes worked for the Deerfield Public Schools for 14 years teaching kindergarten, second and fourth grade classes and as a gifted education instructor. Susie Blau has a Master of Arts degree in behavioral psychology and has worked as a consultant to families, and educators for over 15 years. Elyse Ferdinand holds a degree from Northern Illinois in Early Childhood Education. She has over 15 years of preschool teaching experience. Mary Fisher has been working in the early childhood profession for over 20 years. She has been teaching at the Community House for over 10 years. Emi Kelsey received her training in Early Childhood Education with an area of concentration in music and recently completed a Suzuki Teacher Training program at the Chicago Suzuki Institute. She received her Bachelor of Arts degree from the University of Illinois. Patricia OBrien has been trained and certified by the Protocol School of Washington to present etiquette programs to children, teens and young adults. She has established Manners, Please, providing social skills for every occasion. Carole Pearlman is the founder of The Joy of Creativity art program. Inspiring curriculum, freedom of expression and opportunities to explore many art materials make this innovative program a creative success for each student. Carole holds a BFA in painting and a Masters degree in education and psychology. Find out more about Brandon Seyferth and his guitar classes in our Adult Enrichment section.

Cooking With Amigos Amigos Program Everyday promises to include a fun-filled delicious activity! We will chop, mix, slice, peel and eat! We will do it all while leaning Spanish! From the creators of Camp Amigos, Cooking With Amigos uses a highly controlled vocabulary introduced in conjunction with fun to make recipes. As we wait for our creations to cook, we will have lots of fun playing games in Spanish! The instructor is a native Spanish speaker who has been carefully trained to implement the Amigos Programs curriculum. Please do not register if your child has allergies to egg, soy, dairy, wheat, nuts,\ or berries.

Beginning Guitar: Group Lessons New! Brandon Seyferth This class is for the beginning guitarist. In this class, the student will learn how to apply basic musical concepts such as rhythm, harmony and melody to the guitar. Swordplay for kids Marijke Morich We will be using special plastic fencing weapons with buzzing tips to allow for easier control and increased safety. This class provides a great environment for kids to do one of the things they enjoy most-play with swords! Students will learn basic en garde position, advance and retreats, salute, lunge, extend, beat attack and parry riposte. We will talk about target area and basic rules of fencing. In addition to Olympic style fencing we will also use the weapons for more dramatic war games pitting two teams against each other in an attacking melee. This class will use games to develop speed, reaction time and balance. On the last day of class students will have an opportunity to demonstrate their learned skills to the instructor. Correct execution will allow that student to advance to the next level of Swordplay class. Students who would like to continue to work on these skills are more than welcome to repeat this class until they are comfortable and feel ready to move up to fencing.

Fencing Marijke Morich The Olympic sport of Fencing demands a blend of tactical and strategic thought and a physical execution of those decisions. This has led many to describe the sport as physical chess. Fencing also teaches balance and discipline and even more than this, its fun! Beginning Fencing is for those new to fencing or those who would like to work on the basic skills of fencing. This class will cover basic fencing rules, fencing etiquette, basic footwork, attacks and defenses. Classes will consist of warm up time, footwork time, group lesson and bouting. Equipment is provided.

MEET OUR YOUTH ATHLETICS INSTRUCTORS

Chris Beacom is the President of Illinois Baseball Academy. As a former New Trier Head Coach, Chris has been teaching kids of all ages and ability levels how to play baseball for the past 20 years. Prior to coaching, Chris captained the Northwestern University team and then played professionally in the Toronto Blue Jays organization. In addition to Coach Beacom, Illinois Baseball Academy employs some of the most accomplished and dynamic coaches on the North Shore, many with college and professional playing experience. Hot Shots Sports is a private organization that develops and organizes youth athletic programming. Hot Shots Sports programs stress the pure enjoyment of being involved in athletics and offers the opportunity for children to develop their skills in an environment where everyone plays and reward is given for the effort rather than the result. Participants learn to have respect for each other on and off the court. Hot Shots Sports was founded in 1994 and continues to provide athletic programming in New York City, Denver and Chicago. Hoops4Health is a newly founded organization whose aim is to better the lives of its students. Our goal is to make a positive impact on youth by providing them a fun learning experience in the sport of basketball as well as teaching them how to live a healthy lifestyle off the court. We cater to kids of all ages and skill levels. Our four keys are mind, body, nutrition and discipline. We offer a wide range of programs including basketball camps/clinics, nutrition classes, Tai-chi, Kung-Fu, AAU basketball travel teams, and more. All coaches and teachers are qualified professionals in their field. On-The-Go SPORTS is headed by Chris Martin, former Northwestern University All-American, Chicago Bears Defensive Back, and Big Ten Network football analyst and host.

Lil Pigskins is a national child development program that helps kids discover the game of football. This program is designed for children to learn about football, to have fun playing games with footballs, and to increase their overall health. Our dynamic curriculum is safe, non-aggressive, child focused, and comprehensive. From 18 months to 9 years, we focus on increasing athletic ability, leadership, confidence, and knowledge of the game. FIRST, SECOND AND THIRD GRADE YOUTH BASKETBALL HOUSE LEAGUE


If you have a first through third grade boy or girl interested in playing basketball against other area players without the hassle of traveling to a different location for every game, this program is for them. Participants will learn fundamental skills through weekly practice and games. Adult volunteers coach all teams. First grade games will have youth size basketballs and eight foot basketball rims. Third grade games will have adult size basketballs, 9 foot basketball rims and score will be kept. All games will have staff referees and timekeepers. All participants will receive a team t-shirt and commemorative basketball. 1. Teams will be established according to school (Crow Island, Greeley, Hubbard Woods, Sacred Heart, Faith Hope, North Shore Country Day and more), there will be no evaluation. When filling out your registration form please remember to insert your child/childrens school. Otherwise placement on a particular school team cannot be guaranteed. If there are more than 10 children from a school, multiple teams will be formed. If there too few children from any one school, children will be placed on a team according to village of residence. About Our Karate Program The Winnetka Karate Program follows a systematized belt ranking for all its participants. Traditional Karate is a martial art which emphasizes the principles and applications of weaponless self-defense. Belt Ranking The Winnetka Karate Program follows a systemized belt ranking for all participants. There are 11 belt levels (kyu) before first-degree black belt (shodan). A new student is assigned the white belt rank (eleventh kyu). One must fulfill certain requirements under normal practice conditions and in a test environment to secure progressive promotions up to and including the black belt (dan) levels. When one has acquired a strong foundation in the fundamentals of Karate, he or she can be promoted to the rank of first degree black belt.
The following items are optional and will incur additional costs: uniforms, belt tests and tournaments.

Pee Wee Karate Students learn how to control their energy levels and concentration through study of fundamental Karate principles. Participants will learn how to follow instructions and exercise self control in a fun and exciting environment. Beginner Karate I This is an introduction to Karate. No experience is necessary. (Beginner to Orange Belt/White Stripe) Beginner Karate II Students must have the belt rank of orange or yellow belt to participate. Intermediate Karate Students may join with the permission of the instructor. Karate Curriculum enables the beginner through advanced student to develop an effective form of self-defense. MEET OUR KARATE INSTRUCTORS

Frank Chrzanowski, chief instructor, developed the Traditional Karate Program for the Winnetka Community House in 1989. He has trained in Karate since 1975 and has attained the rank of fourth degree black belt in the Japan Karate Association. He has been featured in articles in the Chicago Tribune, the Sun Times and the Pioneer Press, and has appeared on local and national television programs. Ron Stein has trained in karate since 1981 and is a third degree black belt in the Japan Karate Association. His karate experience extends to Europe and Japan, where he lived for four years, training under many instructors and studying various Japanese martial arts. His karate teaching experience includes The University of Chicago, The University of Illinois at Urbana-Champaign, and the United States Marine Corps in Tokyo. He has been with the Winnetka Community House Karate Program since 1994.

Little Dragons

Taekwondo, one of the newest Olympic sports, is a 2000-year-old martial art, offering a variety of benefits for people of all ages and physical abilities. Students have the opportunity to improve concentration and self esteem, achieve lifelong physical fitness and learn self-defense skills. Classes are taught under the direction of G.Master Ki Hong Kim* (8th Dan World Taekwondo Federation.) A $45 uniform fee is required at the first class. Belt testing is offered through The KH Kim Taekwondo at an additional fee and is held twice a year. All colored belt students are required to have complete sparring gears.

This introductory class is designed to captivate the interest of our youngest students. Skills are developed through gentle instruction and appropriate games for their ability. This class focuses on developing balance, coordination, and respect for the discipline of martial arts training.

SEVEN STAR KUNG-FU

Master Shi Heng Long is a decorated kung-Fu and Tai-Chi Master. He has taught in many different provinces in China including Bejing, Shanghai, Hebei, Shangdong, and Guangzhou. After coming to the USA in 2007 hes taught at The Chicago Shaolin Temple, Health Kick Kung-Fu, and Energy Fitness. In 2009 he taught at The Shaolin Temple Overseas Headquarters in New York and O-Mei Shaolin Kung-Fu in Washington DC. He has studied martial arts since the age of eight at the Shaolin Temple in China. You may know Kung-Fu because of Bruce Lee. However, it actually dated back thousands of years before him. It is an ancient Chinese martial art that brings self defense, confidence, discipline and conditioning to its students. We will teach Northern and Southern styles with Shaolin and animal forms. Kung-Fu training includes stances, stretching, conditioning, meditation/relaxation (Tai-Chi) and forms. All classes are always taught in a safe environment. Students through dedicated training can move up in belt levels. In Kung-Fu students start with no belt and have to earn there first belt which is a white belt. There is a criterion that a student must learn to test for a new belt level. Students go at their own pace in belt progression. Receiving the belt is a great symbol of the learning but most of all it is the personal gain in health, mind, body and spirit which is the ultimate satisfaction. Our professional staff of decorated teachers cant wait to share their passion and knowledge of Kung-Fu. Have fun and enjoy entering the world of Kung-Fu! New students are required to purchase a uniform which includes pants, shirt, and shoes ($60). Belt Test ($25, one time fee for each belt, unlimited tries per belt). MEET OUR CERAMICS INSTRUCTORS

Brenda Lee Ewing has returned to the Winnetka Community House. For almost two decades of teaching ceramics throughout the North Shore, her inspirational instruction to children and adults has been enjoyed by thousands. Her whimsical earthenware sculptures are inspired by the curiosity and energy her students surround her with. Brenda Lees artwork is displayed in many homes across the country. She is especially adept at communicating that talent to those who would like to create with clay. She encourages her students to rediscover their abilities in her classroom filled with intrigue, excitement, and fun. Patty Kochaver has been working in clay for over 30 years, starting with a Raku class in college. She owned and operated Clayworks Studio in Chicago from 1990-1995, taught at the Fine Line Creative Arts Center, and was the Ceramics Studio Coordinator at the Evanston Arts Center from 2000-2010. Patty exhibits her saggar fired work nationally. Alexis Ortiz- After receiving his BFA in 2001, Alexis spent four years in Guatemala and southern Mexico working with Save the Children and Nueva Alianza, both NGOs focused on community responsibility and educational integrity. Upon his return to Chicago he began freelancing with Redmoon Theatre Co. and teaching with Lillstreet Art Center and their outreach program; ArtReach, where, over the next 5 years steadily rose to become their Lead Teaching Artist, Assistant Director and even served as Executive Director for the last two. Now re-focusing his talents and abilities on partnerships and actual art making, both in the classroom and as an independent artist, Alexis joins our team of Teaching Artists and looks forward to getting his hands dirty, again. Greg Schultz received his BFA from Washington University in St. Louis in 1990. He has studied at the graduate level at SAIC. Greg has been studying ceramics at Lillstreet Art Center since 2002 and has also been a studio member there since 2009. Currently, he sells his work online and at his own studio, Schultz Studio Arts. Gabriella Verbovszky holds a BFA from the Cleveland Institute of Art and MFA from the New York State College of Ceramics at Alfred University. Gabriella is currently studying nutrition at the Institute of Integrative Nutrition (NYC). She is a KUSHI Institute Certified Macrobiotic Instructor who has focused attention on the dietary needs of both woman & children after her mother became ill. Prior to the change in diet, Gabriella was a sommelier and received her training in Europe acquiring the highest form of certification including WSET-Wine & Spirit Educations Advisor of Wine Certificate.

Meet our dance instructors


Gwendolyn Britt has been teaching dance at the Winnetka Community House since 1994. She is a graduate of the Indiana University School of Music with a concentration in Ballet and Music. Gwendolyn danced professionally in Ruth Pages Chicago Production of The Nutcracker, for the Lynda Martha Dance Company and with Concert Dance Company of Boston. In Boston she was Associate Director of the Lexington School of Ballet and she is a certified Cecchetti Council of America teacher.

Theresa Evans has been dancing since she was four. She has studied ballet, tap, jazz, modern, hip-hop, ballroom, tai chi, yoga, and Pilates. She danced competitively with Bourn Dancers all over the east coast and was the team captain of the Lake Varsity Competition Dance Team. She had the opportunity to intern as the assistant stage manager for the touring company of Dance Academy South. In the summer of 2007 she became a certified Chair Yoga instructor through the Yoga Center in Canton, Ohio. Afterwards her studies began at the University of Akron. The programs of study she auditioned/applied and were approved for were dance, business administration and a certificate in American Sign Language studies. While attending college, Theresa taught hip-hop, jazz, and stretch/conditioning at a nearby studio.

Patty Marsden has over 25 years of teaching experience. She is a certified dance instructor through the Chicago National Association of Dance Masters and the American Academy of Ballet. As a professional representative for the National Dance Week Organization, she is committed to encourage the growth and development of Dance in America. Patty has functioned as Artistic Director for several suburban dance programs, and as Director and choreographer for Impulse Dance Ensemble. She was the dance director at St. Viator High School and taught Master classes for several other high schools. Patty has taught at several private studios and park districts in the Northwest suburbs and on the North Shore.

Meghann Mueller received both a BFA in Dance and a BA in History from Texas Christian University in Fort Worth, Texas. She began her early dance training at the Evanston School of Ballet under Kerry Hubata and Laura Wade. During summers, Meghann studied at the Joffrey Ballet and on scholarship at the Milwaukee Ballet. While at TCU, she had the opportunity to perform works by TCU faculty members, as well as several guest artists including Elizabeth Johnson, Dana Nicolay and TCU Choreographer in Residence Fernando Bujones. In the spring of 2006 Meghanns work, Narrow Anguish, was performed at the American College Dance Festival in the adjudicated concert. In 2007 Meghann formed Project606 Dance with former classmate Maureen Caspers. The company has performed throughout Chicago including at the revered Hamlin Park Fieldhouse and at the Atheneum Theater as a part of Dance Chicago. E. Marc Nevins is Winnetka native and currently serves as Artistic Director of Ballet Entre Nous, a company he founded in 1985. He has choreographed and staged over 100 works for the company including an annual production of The Nutcracker! at Winnetka Community House. His work has been seen all over Chicago. has appeared with such companies as American Ballet Theatre, the Joffrey Ballet, Ballet West and locally, Evanston Concert Ballet, Oak Park Ballet, Masi Ballet Chicago and Lake Shore Dance Theatre. Mr. Nevins has taught ballet extensively throughout the Chicagoland area for nearly 30 years and has been teaching at Winnetka Community House since 1987.

Pre Ballet This class serves as a stepping stone into the beautiful art form of classical ballet. It includes barre exercises, center work and stretching, but also includes creative movement with princess wands, beanbags and scarves. Students learn classical ballet positions of the feet and arms, basic ballet steps and French terminology.

Creative Paws, Inc. was founded in 2001 by Ashlee Trotter, BS, CPDT, and provides group classes and private lessons for dog obedience training and behavior problems. Ashlee uses positive reinforcement training methods to improve the relationship between dog and handler by providing an effective means of communication and understanding. DOG OBEDIENCE
Puppy The purpose of Puppy Class is to provide a positive environment for socialization of puppies to a wide variety of people as well as to each other. Early socialization has been shown to greatly reduce the incidence of both human-directed aggression and dog-dog aggression, as well as anxiety and fear based behavior problems. Additional topics will be discussed such as addressing the common puppy issues of housetraining, mouthing, leadership, and preventing behavior problems. Puppies will also learn sit and begin recall training (coming when called). This class is appropriate for puppies ages 24 months old. All dogs must have received at least 2 rounds of vaccinations prior to attending class. If your dog has not yet received the second round at the start of the session, please do not bring him/her to class. Instead, plan to attend class without your puppy until the second round of vaccination has been administered. Children training dogs must be at least 12 years old. No retractable leashes allowed. Obedience 101 Obedience 101 teaches the basics of obedience: sit, down, stand, Recall (coming when called), stay, and heeleverything your companion dog needs to know! Both verbal commands and hand signals will be used. Dogs of any age may take this class. Positive reinforcement techniques will be used, so please bring food rewards or a small toy with you to class. Children training dogs must be at least 12 years old. No retractable leashes allowed. Obedience 102 Obedience 102 is focused on honing the skills of Obedience 101. Additional levels of difficulty and distractions will be added. Dogs of any age may enroll provided they have a basic knowledge of basic obedience commands: sit, down, recall (coming when called), stay, and heel. Children training dogs must be at least 12 years old. No retractable leashes allowed. Canine Good Citizen (CGC) Canine Good Citizen (CGC) class covers the 10 aspects of the AKC CGC test, the only AKC certificate available to all dogspure- and mix-breed. The CGC program is designed to encourage dog owners to train their dogs to be Canine Good Citizens and includes the following: Accepting a friendly stranger; Walking on a loose lead; Sitting politely for petting; Allowing basic grooming procedures; Walking through a crowd; Sitting and lying down on command and staying in place; Coming when called; Reacting appropriately to another dog; Reacting appropriately to distractions; Calmly enduring supervised separation from the owner. The CGC test is administered on the last night of class. Children training dogs must be at least 12 years old. Dogs must wear a flat buckle collar for the test, no training collars, head halters or body harnesses are permitted during the test. No retractable leashes allowed.

Brandon Seyferth is a critically acclaimed songwriter and composer who is accomplished on the guitar, blues piano, drums and voice. Where Arts, Learning and Community Come Together! Founded in 1924, the North Shore Art League is a non-profit organization offering art classes for adults and children. As the only art center located in Winnetka, the NSAL not only offers art education, but also lectures, workshops, field trips and an ongoing series of gallery shows to the entire North Shore community. Located on the second floor of the Community House, classes are held in an historic, professional art studio featuring skylights and a large stone fireplace as well as a well-equipped print room and ceramics facility. Falun Gong, also called Falun Dafa, is a powerful body and mind exercise system. It contains five sets of exercises that work on the energy circulation of the body. The movements are smooth and graciously flowing, and can be learned easily by any age group. The mind exercise is applying Zhen (truthfulness), Shan (compassion) and Ren (Forbearance) into daily life. The Interfaith Housing Center is a 39 year old notfor profit organization working to assure a housing market open to people of all ages and income levels, regardless or race, religion, family composition or any of the other legally protected classes. Interfaith promotes integrated, just communities through its four major programs: Homesharing where home providers are matched with individuals seeking affordable and safe housing; Fair Housing, which includes housing discrimination and landlord/tenant complaint investigation, housing information and referral, community education and assistance to municipalities; Predatory Lending Prevention, which helps homeowners who may be victims of unfair lending practices; and Affordable Housing Advocacy, which includes the building of support for affordable and fair housing in our communities. Interfaith works in 16 northern suburbs and its membership includes local residents and more than 60 civic organizations and congregations.

The mission of the Winnetka Youth Organization (the YO) is to foster individual development in junior high and high school youth on the North Shore by providing adult-to -youth mentoring, prevention services, and opportunities for leadership, citizenship, and education within a safe, informal environment. Member registration opens July 11, 2011, and will be processed on a first come, first served basis. Membership does not guarantee placement in a class. All non-members will be able to register July 18, 2011. Registration received through the mail, fax, or in person will be processed by 5:00 p.m. the next business day. The best way to ensure that your registration is processed immediately is by using our online registration at winnetkacommunityhouse.org

Refund Policy

Membership Information
Early registration: Members are eligible to register for all classes one week prior to the start of seasonal registration. Early registration can be done via the web at winnetkacommunityhouse.org, fax or in person at the Winnetka Community House. Please note: early registration does not guarantee a spot in a class. Who is eligible for benefits? All family members in your household are eligible for membership benefits. When registering for membership, just list your family members, and they will be included for the program registration benefits automatically when registering for classes. Membership renewal: All memberships expire on the June 30 of each year.* Members receive a renewal notice about one month prior to the expiration, so the membership can be renewed for the next year. Refunds for memberships will not be available.

Full refunds will be issued for courses that are cancelled by the Winnetka Community House due to insufficient registration. A refund, less a 10% service charge (not to exceed $25), may be granted if the refund request is received five full working days prior to the start of the program. No refunds will be allowed after this time unless a medical condition develops and a written explanation from a physician on his/her letterhead is presented. Due to advanced reservation requirements, no refunds for trips, outings, or special events will be given.

Prorated Policy
Classes may be prorated if you would like to enroll in a class after it has started, provided the instructor has space, and allows for late registration. The supervisor must approve the prorated amount.

Inclement Weather
The Winnetka Community House will not hold classes on days when the Winnetka Public Schools have cancelled classes due to inclement weather.

Religious Holidays
The Winnetka Community House makes every effort to avoid scheduling programs or events on religious holidays. Anyone affected will be offered the option of attending a session of the same program on a different date as a make-up, or receiving a prorated refund of the series fee for the date missed. The Winnetka Community House is not responsible for the supervision of children on site prior to or remaining on site after the established program times. The Winnetka Community House is handicapped accessible. If you or any member of your family has special needs, please see our Executive Director. The Winnetka Community House receives no tax dollars. We are supported by our programs, the Antiques and Modernism Shows, our annual campaign, and donations. The Winnetka Community House assumes no responsibility and carries no accident or medical insurance for injuries or accidents at programs or activities on Community House property. It is advisable to review your own personal health insurance plan to be certain that you and your family have proper and sufficient insurance coverage.

How To Register
To register for membership, simply list all eligible members on the registration form. Then check the box and add the membership fee in with your registration. Register for programs online at winnetkacommunityhouse.org or on the enclosed registration form. Please print clearly. Read and fill out form completely. By listing your email address, you agree to receive email communications from the Winnetka Community house. We will never sell your address, but love to keep in touch with our users to give news about programs, events, and special offers from the Winnetka Community House. If you do not wish to receive these emails, simply unsubscribe. Sign the registration form. Unsigned forms will not be processed, but will be returned to you. This will delay processing your registration. Registration forms may be mailed or faxed. Registration by telephone are not accepted.

Brochure Errors
Complete details of program policies, procedures, and guidelines are sometimes inadvertently omitted. Errors in days, times, registration requirements, and fees may occur in the brochure. When such errors occur, the staff will advise you of the change as quickly as possible. We thank you for your understanding.

Credit Cards
You may use your credit card for all programs which appear with a class number or for a Fitness Centre membership. Fitness Centre memberships are obtained at the Fitness Centre only. Be sure to fill out the credit card information completely and sign the form. We do not take telephone registration. However, if you are paying by credit card, you may fax your registration to (847) 446-0609.

Cancellation of Classes
Prior to any class, registration is reviewed to determine if the enrollment is sufficient to hold the class. Register early! Do not wait until the day of the first class to register. You may find the class has been cancelled due to low enrollment.

STEPHEN DUSTMAN MEMORIAL SCHOLARSHIP FUND

The Winnetka Community House has established the Stephen Dustman Memorial Scholarship Fund to any resident who would like to take part in a Winnetka Community House activity, but is unable to render full payment. Persons interested may submit their need in writing to the attention of the Executive Director for confidential review. If further information is desired, please call our main office. The Winnetka Community House is committed to conducting its recreation programs and activities in the safest manner possible, and holds the safety of the participants in the highest possible regard. Participants and parents registering their child in recreation programs must recognize, however, that there is an inherent risk of injury when choosing to participate in recreation activities. The Winnetka Community House continually strives to reduce such risks, and insists that all participants follow safety rules and instructions which have been designed to protect the participants safety. Please recognize that the Winnetka Community House does not carry medical accident insurance for injuries sustained in its programs. The cost of such would make program fees prohibitive. Therefore, each person registering himself\herself or a family member for a recreation program/activity should review his/her own health insurance policy for coverage. It must be noted that the absence of health insurance coverage does not make the Winnetka Community House automatically responsible for payment of medical expenses. Due to the difficulty and high cost of obtaining liability insurance, the agency providing liability coverage for the Winnetka Community House REQUIRES the execution of the following Waiver and Release. Your cooperation is greatly appreciated. Please read this form carefully, and be aware in participating in the program(s) listed on this page, you will be waiving and releasing all claims for injuries you might sustain arising out of the activities of this program. Waiver And Release Of All Claims As a participant in the program, I recognize and acknowledge that there are certain risks of physical injury, and I agree to assume the full risk of any injuries, including death, damages or loss, regardless of severity, which I may sustain as a result of participating in any and all activities connected with or associated with such program. I agree to waive and relinquish all claims I may have as a result of participating in the program against the Winnetka Community House and its officers, agents, servants and employees. I do hereby fully release and discharge the Winnetka Community House and its officers, agents, servants and employees from any and all claims from injuries, including death, damages or loss which I may have or which may accrue to me on account of my participation. I further agree to indemnify and hold harmless and defend the Winnetka Community House and its officers, agents, servants and employees from any and all claims from injuries, including death, damages or losses sustained by me or arising out of, connected with, or in any way associated with the activities of the program. In the event of emergency, I authorize Winnetka Community House officials to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed necessary for my immediate care, and agree that I will be responsible for payment of any and all medical services required. All participants agree that any photography taken while participating in a class, special event or use of a facility may be used for promotional purposes the Winnetka Community House. Participants agree to receive emails from the Winnetka Community House. I have read and fully understand the aforementioned Program Details, Waiver and Release of All Claims and Permission to Secure Treatment. Registration form will not be processed without waiver signature.
